  • Why this form is required: 

    NDIS Code of Conduct: Providers and workers are required to deliver supports in a safe and competent manner, acting with integrity and promptly addressing concerns that may impact the quality and safety of supports provided to people with disability.

    Conflict of Interest Guidelines: The NDIS Practice Standards advise providers to implement strategies that prevent conflicts of interest, which can include ensuring that no single worker holds excessive influence over a participant's care.

    Operational Guidelines on Informal Supports: The NDIA recognizes the importance of sustaining informal support networks and advises against arrangements where a participant relies solely on one individual, as this can compromise the participant's wellbeing and the carer's capacity to provide ongoing support.
