2025-26 Player Equipment Package Sizing Form
Each player's contract includes an equipment package. Please follow the steps below to enter the sizing requirements for your equipment package as outlined in your player's contract.
STEP 1 - Equipment & Apparel Sizing Process
There will be no in-person fitting sessions at BSA. For all sizing references, you must use the sizing charts provided at the "Jersey & Apparel Sizing Charts" link below.

STEP 2
Once you have all your player's sizes, scroll down and enter your player's full name as entered on your player's contract in the "Enter Your Player's Full Name" field, and click the "Click To Auto-Populate Fields" button. This will auto-populate the details of your player's Team, Email, and equipment package.
NAME EXAMPLE: Case-Sensitive > Jon Doe
Make sure to capitalize the first letter in your player's First and Last names. Your player's name must be entered exactly as shown on your contract, with a space between the first and last name.
STEP 3
If you have issues with your player's name auto-populating, please email us as soon as possible, and do not complete this form. DO NOT change the Team name or email on the form, as it will auto-populate.

STEP 4
Select three options for your player's Jersey number. Priority will be determined as follows:
1) The returning player who wore that number last season will get priority.
2) If no returning player wore that number, the player who completed and submitted this form first will get priority.
3) Note, you must provide three different numbers. If you submit the same number all three times, your priority will be moved to last.
STEP 5
Select the appropriate information and sizes for your equipment and apparel.

STEP 6
Sign and click "Submit". You will receive a confirmation email.
Please note that you must have completed your first tuition payment and be in good standing for the order to be processed. Your tuition must be paid in full to receive the equipment once it arrives.
