Final destination MUST be Chicago, IL (O'Hare or Midway Airport). This service is exclusively available for arrivals into Chicago.

Instructions:

1. Please fill out this form for EACH 18+ adult passenger.

2. Each passenger should have a copy of their passport/green card/visa kept in their possession BEFORE the date of travel.

3. For EMERGENCIES AT THE AIRPORT ONLY, please text (630) 930-2987 with the traveller's full name, passport number, nationality, flight departure/arrival/airline details, visa details, and emergency contact person's details.

**Notify your emergency contact at the following checkpoints:

(1) when your flight lands

(2) when you approach customs

(3) once you have cleared customs
