We will determine the best track. Courses are linked above under the heading of this form. Also, kindly list Piano as a choice for each student wishing to take piano lessons and specify your preferred day and time. *

I acknowledge that pictures will be taken throughout the course of the academic year. It is my responsibility as the parent/guardian to alert the one taking photographs that I do not want my child(ren)'s picture posted or shared. Pictures may make it into the Above Reproach Yearbook. I do not hold Above Reproach Academy liable for any pictures taken or shared of my child. I understand I must purchase a yearbook if we would like to have one.
