Hello! I'm Alyssa, and I’m so honored that you're interested in getting inked by me! (ෆ˙ᵕ˙ෆ)♡

Before we dive into booking your appointment, please take the time to thoroughly read and fill out this form as best as you can. It helps me keep things smooth and easy-peasy for both of us!

Once you submit, I'll reach out via email (from "alyssa@modoink.com" ONLY; please be cautious of scams). I will confirm your design, offer some dates/times, and provide an estimated cost.

Let's talk details:

You must be at least 18 years old to get tattooed, regardless of parental consent—and please don't forget your ID. Sorry, no exceptions!
All appointments require a non-refundable deposit to lock in your appointment. Your timeslot will be held for 24 hours and the deposit must be paid within that time. Your deposit will count towards the final cost of your tattoo. Your appointment is not confirmed until the deposit is received. Bookings are first come first serve. In the case of reschedules, deposits are valid for 1 calendar month from your original appointment date. After that, they're forfeited.
If you need to reschedule, please let me know at least 24 hours before. Life happens; I get it! You can still use your deposit to rebook. But, if you're a no-show, your deposit is void and you'll either need to put down a new one to reschedule or, depending on the situation, can no longer book with me.
Running late? No worries if it's 5-10 minutes. If you're 30+ minutes late with no contact, your deposit will be forfeited and the appointment cancelled.
Custom designs are shown in-person at the studio on the day of your appointment. Small tweaks are acceptable, but big adjustments may require us to push your appointment back and may affect the quote. I do not email mock-ups or drafts of the design beforehand, unless otherwise discussed.
Please allow up to one week for me to respond. I juggle emails and DM's inbetween clients so it can take me a second! I read every single entry, so if you don't receive an email back within a week, please check your spam folder or follow up with me in an email or Instagram DM. Please do not submit another form for the same tattoo inquiry.
I am not responsible for making sure you make it on time to plans you have scheduled after the appointment, so please set aside the whole day. Art is best when not rushed!
Make sure to eat something within 2 hours before your tattoo appointment and refrain from drinking alcohol or taking blood thinners for at least 24 hours before your appointment.
Please wear clothes that allow easy access to the area that will be tattooed. Dark colors and black are recommended in the case of accidental ink spillage. Tattoo ink is difficult (if not impossible) to get out of clothes.
I offer 1 free touchup within 12 months of your tattoo appointment. Finger and foot tattoos are not eligible for free touchups.

FAQ:

How much will my tattoo cost?
This depends on size, detail, and placement. I will provide an estimated cost during our email correspondence. Currently, I charge a flat rate per piece; I do not charge hourly.
Are there any concepts you won't do?
Anything pertaining to hate-speech, racism, homophobia, etc. Otherwise, I'm open to your concepts, but I do aim to take on designs that fit my style to ensure that we both end up with art that we love!
Cash, card, or Venmo/Zelle?
Deposits are through Zelle or cash in-person. The rest of the tattoo can be paid for with Venmo, Zelle, or cash. Please note, there is no ATM in the shop.
How do I send a deposit?
I will send a how-to blurb through email. Deposits are through Zelle only or cash in-person.

Please disclose, if any of these apply to you; you can opt to talk in person if preferred, and our shop consent form goes over these in more detail: Allergies (ink, ink brands, latex, soap, etc.), skin conditions, (eczema, psoriasis, scarring, sunburns, fresh tan, etc.), blood disorders (hemophilia, clotting issues), autoimmune diseases (ex: diabetes, hepatitis)(can affect healing & ink retention), heart conditions, accutane, pregnant/breastfeeding, accessibility requirements, if you need to bring along a support person, this is your first tattoo, etc.

I confirm that I am 18 or older, and understand that submitting this request does not guarantee an appointment. A non-refundable deposit is required within 24 hours to secure the appointment. Without this deposit, the appointment is not confirmed. Cancellations or rescheduling must be made at least 24 hours in advance; otherwise, an additional non-refundable deposit will be required to reschedule. I will not arrive to the appointment under the influence of alcohol or drugs. Last-minute changes to designs may affect the original quote. I agree to follow aftercare instructions and recognize that if I fail to do so, my tattoo may heal incorrectly. I give permission for photos of my tattoo to be used for portfolio/social media purposes.
