This form is for tattoo requests 12" or smaller. Forms submitted here will be automatically added to the waitlist for Indianapolis appointments. I will give directions on how to submit forms for guest spots when I announce them - forms that do not follow directions or are for places I'm not going will be ignored. Ty!

Do you have existing tattoos that you would like this tattoo to be near, or would you like this tattoo to be a gap filler? Basically : will I have to fit this tattoo around or between other tattoos you already have?
If you are comfortable, please provide a picture of the area in question. If you do not have any existing tattoos in the area I do not need a photo.

This works best if someone else takes the picture or you take a picture in the mirror. Have the area totally relaxed and take the picture straight on. You can also use the Markup tool on an iPhone to outline the area!

My hourly rate is $175/hr. Typically palm-size tattoos fall around $300/$350, and hand-size tattoos fall around $450/$500.
