  • Introduction

    We are community members at organizations in Seattle and South King County. We're gathering information about the root causes and risk factors of gun violence. We want to tell the story about how gun violence affects people in our communities. We're working with our partners to develop clear, community-informed recommendations and solutions.

    The organizations working on this project include:

    • 4C Coalition
    • CHOOSE 180
    • Community Passageways
    • Glover EmpowerMentoring Program
    • Seattle CARES Mentoring
    • The Alliance for Gun Responsibility


    We are asking for input from the following members of our communities:

    • People who have a direct connection to gun violence, either as a victim or a user of guns
    • Family members or loved ones of someone with a direct connection to gun use or gun violence
    • Staff or service providers who work with people who have a connection to gun use or gun violence.

    Participants who complete this survey will receive $50 as a thank you for their time. If you choose to receive payment, we will collect your contact information at the end of the survey. Your contact information will be stored securely and kept separate from your survey responses to protect your privacy.

    Your privacy and confidentiality are our top priorities. Your responses to this survey are anonymous. You may choose to stop taking the survey at any time. Anything we quote will remain anonymous and will not connect back to you.

    The information you share will inform our recommendations to the City of Seattle. We will analyze all responses and share them in a dashboard for the community. Our goal is to make all communities safer and free of gun violence.

    This section will collect basic background information such as your race/ethnicity, age, gender, and ZIP code. This helps us understand how gun violence impacts different communities and ensures that diverse voices are represented.
  • Directly Impacted

    This section is for individuals who have personally experienced gun violence—whether as a victim, survivor, or someone who has caused harm. Your insights will help us better understand the lived realities and needs of those most directly affected.
  • Family Member, Friend, or Loved One

    This section is for people who have a close relationship with someone impacted by gun violence. We want to hear about your experiences and perspectives, as the ripple effects of violence often extend beyond the individual directly involved.
  • Staff/Providers

    This section is for professionals who work with individuals or communities affected by gun violence (e.g., case managers, outreach workers, healthcare providers, educators). Your input helps us understand systemic challenges, service gaps, and opportunities for support.
