School-Run Extra-Curricular activities during After School Care (ASC)

This form is to be completed if your child is participating in an extra-curricular activity that is being run by Wishart State School. This will be a teacher-led activity.

In completing this form, I am giving permission for my child, noted below, to be collected by the activity teacher from Wishart Outside School Hours Care during the ASC session in order to take part in afternoon activities outside of the Service.

I understand that I must supply the following information to WOSHC in order to comply with regulations.

• Activity type
• Location were activity is being conducted
• Start & Finish dates
• Start & Finish times
• Collection/Pick up requirements

I understand that I must complete this information for each day that my child is attending a school-run afternoon activity being conducted during after school care.

I understand that if the form is not completed properly I will be contacted to complete a new form. 

I understand that I need to complete seperate forms for activities conducted during before school care and after school care sessions, as well as acitivites completed by externally run providers.

I understand that a new form is required EACH TERM that my child attends an activity.

By completing and signing this form, you agree to the following:

• I have communicated to my child that they are to go first to the relevant WOSHC sign-in area so that they can be signed in to ASC.
• I have communicated to my child that they are not to leave the service before first confirming they can leave and have been signed out by an educator.
• I agree to the teacher running the activity collecting my child from the WOSHC office, and will return my child to the WOSHC office at the end of the activity, if required.
• I understand that whilst away from the service and at the activity; that my child will not be under the care of WOSHC.
• I understand that responsibility for my child will once again be that of the service, once my child has finished the activity and is returned to WOSHC by the teacher.
• I understand that if my child is booked in, I will be charged for the time my child is away from the service.
• I undertake to notify the service if, and when, this arrangement changes and at that time I will complete a new extra-curricular activities release form.
• I undertake to notify the service in writing if my child is no longer attending any of the above extra-curricular activities.
• If my child is absent from school, I will notify the service as per usual and my child will be marked absent. Should your child not arrive to WOSHC to sign in, the normal procedure of following up on your child’s safety will take place and a non-communication fee will be added to your account for failure to notify.
• I understand that WOSHC staff are not responsible for delivering and collecting my child/ren from external activities.
• I understand that should the service be required to follow emergency procedure policy (eg Lockdown or Fire Evacuation), that my child will not be able to leave the service until the emergency procedure policy has been finalised. It is not the service's responsiblity to follow up with the teacher who is collecting from the service, and therefore my child may miss this activity due to the emergency procedure.
