    We help people care for their pets during times of crisis.
    This form helps us get to know you and your pets so we can offer the right support.
    All answers are confidential.

    Important Information About Our Pet Pantry

    • Free Pet Food & Supplies: Available to families in Utah. All items are donated and provided based on availability. They’re meant to supplement, not fully replace, your regular pet food. This is per family not per household. 
    • No Proof of Income Needed: If you say you need assistance, we trust you. No financial info required.
    • Timeframe for Assistance: You can receive pantry support for 6 months. After a check-in, support may be extended up to 12 months total, with further help offered case-by-case.
    • Spay/Neuter Requirement: All pets must be spayed or neutered within 6-12 months of starting the program. We may be able to help cover the cost.
    • ID Required: You’ll need to show some form of identification. It doesn’t have to be government-issued. It can be a photo ID, a letter with your name/address, or a caseworker note.
    • No New Pets: You can’t add new pets to the application during the first 6 months unless there’s a change in circumstances and it’s approved.
    • Picking Up for Someone Else: We’ll need their info on file.
    • Phone Intake Required: A 5-10 minute call is part of the application process. Let us know if you need a different format. We’re happy to accommodate.
  • PICK-UP SCHEDULE

    Tuesday, November 4, 8am-9am
    Thursday, November 6, 6pm-7pm
    Saturday, November 8, 12pm-1pm

    You may pick up food once a month.

    No appointment necessary; however, you will need to complete the application and intake call before picking up food. 

    If you walk in without having completed an intake, we will be unable to provide anything.

    We ask these questions to help you and your pet in the best way we can. Your answers also help us improve our program and show funders how our services make a difference.

    We will never share or sell your personal information without your permission, unless the law requires it. Your information is stored safely and kept private.

    When we share information for reports, we remove your name and any personal details so no one knows it’s you.
