  • 2025 Bergen Youth Task Force - Session 3 Sign Up

    Calling all Bergen County students in 8th through 12th grade & adult advisors! You are invited to the final of three sessions of the Bergen County Youth Task Force. Join us on Wednesday, May 21st from 6:30 to 8:00 pm, held at Children’s Aid and Family Services (200 Robin Rd in Paramus). You'll get a chance to meet youth from across Bergen County, learn things to bring back to your school and community, and make an impact on the world around you!

  • I, the undersigned, hereby give my child permission to attend the Bergen County Youth Task Force session on May 21st from 6:30 to 8:00pm.

    I agree that they will attend the session at Children’s Aid and Family Services located at 200 Robin Rd in Paramus. I understand that the Partners in Prevention staff will be present at the event and that after that time, my child will be dismissed from the event. I understand that the Partners in Prevention staff is not responsible for supervising my child after the programming ends, and it is my responsibility to find out whether or not my community is sending an adult chaperone and to obtain chaperone contact information.

    I give permission for photographs/video footage to be taken of the participant (either in person or online), and for photographs/video footage in which the participant is included to be used for purposes of publicity by LMTI and NCADD-Hudson/Partners in Prevention. This includes publication of pictures/video on LMTI websites social media outlets.

    Should an emergency arise, I understand that I will be notified as soon as possible, and I give permission for the attending medical professional to administer treatment to my child. I understand that I am responsible for payment of any medical treatment via insurance or other means. I will hold harmless the Lindsey Meyer Teen Institute and NCADD Hudson/Partners in Prevention, Governor's Council on Alcohol and Drug Abuse, Bergen County Department of Health Services, and Bergen County Municipal Alliance.
