      1. Conduct a full walkthrough of the premises to identify any potential hazards.

      2. Take note of any new hazards as well as any ongoing or unresolved issues.

      3. Once the walkthrough is complete, return to your workstation and record all identified hazards on this form.

  • How to record the Hazards you have identified:

    • Multiple hazards can be recorded in one submission – click on the Add button
    • Specify the type, provide a brief description, and indicate the exact location (e.g., "Treatment Room 2", "Reception Area")
    • Use the Level of Seriousness scale from 1 (Low) to 5 (High) based on your judgement.
    • Once completed, your form will be reviewed by management. You do not need to notify anyone separately.
