I, the undersigned, acknowledge and agree to the following terms and conditions:

1. Consent to Treatment

I consent to the facial treatments provided by Broadway Skin Studio. I understand that the esthetician will perform services based on the information I provide, and these treatments may include cleansing, exfoliating, masks, chemical peels, massage, extractions, and application of serums or other skincare products.

2. Health History

I confirm that I have provided accurate and complete information regarding my health, allergies, medical conditions, and any previous skin treatments or procedures. I understand that withholding information regarding my health could result in injury or a negative reaction to the treatment.
It is my responsibility to inform the esthetician of any changes to my health history.

3. Risks and Side Effects
I understand that while esthetic treatments are generally safe, there are risks and potential side effects involved, including but not limited to skin irritation, redness, allergic reactions, scarring, hyperpigmentation, or breakout flare-ups. I agree to promptly inform the esthetician if I experience any discomfort or unusual reactions during or after the treatment.

4. No Guarantee of Results
I acknowledge that the results of any facial treatments cannot be guaranteed. Each individual’s skin may respond differently, and optimal results may require multiple treatments or maintenance sessions.

5. Skin Sensitivity and Product Reactions
I understand that the esthetician will assess my skin type and choose appropriate products and treatments. However, I am aware that reactions or sensitivities to products may occur, and I agree not to hold Broadway Skin Studio, the esthetician, or any affiliates liable for any reactions or adverse effects that may occur.

6. Home Care & Results

I acknowledge that facial results may vary depending on my skin condition, consistency of care, and home routine. Broadway Skin Studio is not responsible for results affected by my home care, product misuse, or lack of follow-up treatments.

7. COVID-19 or Other Health Considerations
I acknowledge that Broadway Skin Studio follows all necessary hygiene and sanitation protocols to prevent the spread of COVID-19 and other communicable diseases. By attending my appointment, I confirm that I do not have any symptoms of illness, including fever, cough, or shortness of breath. I agree to reschedule if I am feeling unwell or have been in contact with anyone who is sick.

8. Right to refuse service

Broadway Skin Studio reserves the right to refuse service for reasons including but not limited to medical concerns, inappropriate behavior, or policy violations.

9. Data Privacy 

I understand that my personal information will be kept confidential and only used for purposes related to my treatment at Broadway Skin Studio.

10. Release of Liability

I release Broadway Skin Studio, its staff, and affiliated parties from any liability, claims, damages, or injuries that may result from my treatment or the use of services provided. I agree not to hold Broadway Skin Studio responsible for any injury, allergic reaction, or other adverse effects that may arise from any facial or skincare treatments received.

By signing below, I acknowledge that I have read, understood, and agree to the terms and conditions outlined in this waiver and consent form. I consent to the treatments being provided and assume full responsibility for my health and well-being during my visit.

 

I understand that I must provide at least 24 hours' notice to cancel or reschedule my appointment. Failure to do so will result in a 50% cancellation fee for the scheduled service. If I fail to show up for my appointment without prior notice, I will be charged the full service amount.

By signing below, I confirm that:

1. I have read, understood, and agreed to all the terms and conditions outlined in this Waiver & Consent form.
2. The information I have provided is accurate and complete to the best of my knowledge.
3. I understand the potential risks, side effects, and the importance of following the aftercare instructions provided by Broadway Skin Studio.
4. I acknowledge that any questions I had about the treatments or the form were answered to my satisfaction.
5. I consent to the facial treatments and understand the policies outlined in this form, including cancellation, photography, and health considerations.
