    If you have any combination of Youth Camper or a Kids Camper they can be on the same form!

    Just remember, they must be from the same household or immediate family.

  • Universal Release Form

  • Church policy states that all participants must fill out a Universal Release Form prior to attending any special or off campus events/activities.  The Universal Release Form provides the church with medical and insurance information if an accident were to occur involving your child. These forms must be updated annually. Current forms are good from October of 2024 to present. 

    If you need a Universal Release Form don't worry. Once you submit this registration form you will receive a confirmation email that has a Universal Release Form attached. Please return it to the Education Office at the church (either in person, mail or email) as soon as possible or no later than the first day of your child's camp.

  • After you click the SUBMIT FORM button you will receive a message confirming your registration. On that confirmation message you will have the option to:

    • Download a Universal Release Form
    • Register additional Campers
    • Make an online payment for the amount below
  • If you make an online payment for camp (regardless of which camp(s) you are paying for), on the payment page on the church website do the following:

    1. Go to fbcchattanooga.org
    2. Click on 'Give' at the top of the page
    3. Input the payment amount (see above)
    4. CLICK on the dropdown menu
    5. Choose: FBC YOUTH & KIDS CAMP OR Music & Arts Camp
      1. Note: You will have to make two payments if signed up for Youth/Kids Camp AND Music & Arts Camp
    6. Fill in your card/payment information
    7. CLICK the SUBMIT button
