Program Book Ad Form - due April 20th
Hi dance parents! 1 group picture will be included in the program book, but should you wish to also include an individual ad, you can fill out this form to do so. Please read each section carefully and review your spelling before submitting your ad form. You will not be prompted to pay for your ad on this form. Your ad charge will be posted to your account and run with the may draft. If you would prefer to pay by cash or check, just be sure to stop by the office before the draft runs. Thank you, parents! Remember, your ad can include logos or business cards if being sponsored by a business. You can also include more than 1 image on your page, for an additional $10. This could be multiple pictures from this year, or photos from years past if celebrating 10 years of dance for example. Ads can only be designed by our graphic designer in order to ensure continuity within the program book. If uploading any documents, please only include singular, unedited images.
