Photographic & Volunteer Release
I hereby acknowledge that the information given above is accurate and I give permission to the community for the use of my photographs that are taken by staff in order to use for social media.
I, on behalf of my child, hereby release and waive any and all claims for damages, injury, or death against either Anchor of Hope Foundation or Dayspring Presbyterian Church including their officers, directors, employees, agents, independent contractors, and staff that may accrue to me or my child as a result of my child’s participation in the 2025 Autism Awareness race and festivities, and agree to indemnify, protect, and hold harmless Anchor of Hope Foundation or Dayspring Presbyterian Church from any claim or liability whatsoever, including, but not limited to, personal injury, property damage, court costs, and attorney’s fees, however caused, as a result of my child’s participation in the 2025 Autism Awareness Festival.
I further grant full permission to the above mentioned sponsors, organizers and or agents authorized by them, to use any photographs videotapes, motion pictures, recordings or other record of the event for any reasonable purpose.
In consideration of my participation at the Anchor of Hope/Dayspring Presbyterian Church race event, I am aware of the existence of the risk on my physical appearance to the venue and my participation in this activity of the may cause injury or illness such as, but not limited to Influenza, MRSA, or COVID-19 that may lead to paralysis or death.
