    Soccer Hall of Fame - Ontario Nomination Form

  • The Selection Panel of the Soccer Hall of Fame – Ontario (SHOFO) very much welcomes nominations for induction into the Hall. Nominations will be accepted from any person or organization by completing the Nominations Form.

    Nominations will be evaluated annually, after the annual deadline of July 31st.

    The Board of Directors of SHOFO will make the final decision on all nominated candidates based on the submitted criteria.

  • In your opinion, has the committee excelled in soccer, brought great credit to the sport, and is respected in the soccer community in Ontario
  • In your opinion, are there achievements noteworthy on a "best in class "basis

  • Criteria For Players

  • Have they played the game with excellence at the professional, youth or adult amateur level with a minimum of 10 years?

  • Criteria For Builders, Coaches & Referees

  • Have they served soccer with distinction over an extended period for a minimum of 10 years in their role?

  • Criteria For Teams Of Distinction

  • Have they accomplished significant feats in the international, national or provincial level?

  • Criteria For Organizations Of Distinction

  • is this a Ontario soccer club or organization that has made a positive impact on the game at a national, provincial or community level for a minimum of 50 years?

    To be eligible nominees must have been born or raised in Ontario or moved to Ontario permanently before or during their activity for which they are being recognized
  • Has the nominee been active in soccer in Ontario for the last five years as a youth and/or an adult?
  • Was the nominee born in Ontario?
  • Were they raised in Ontario?
  • If moved to Ontario, have they become a permanent full-time Ontario resident?
  • If they move to Ontario, were they a full-time Ontario resident during at least some of their time of their achievements being recognized?

  • Other Considerations

  • By next November 1st, will the nominee have been retired for at least five years from the calibre of play/involvement for which they are being honoured?
