Introducing the purpose of this form 

The NMSDOH Collaborative's Data & Technology workgroup is launching this form in an effort to build off of the Gravity Project's work and recommending that the HCA use SDOH domains for the referral system, which will require the least amount of transition for those transitioning into the system. The use of domains is instead of a standard screener for instance. However, for some organizations who do not already have a screener at all in place, they may have to adopt one. In this form, we are seeking input on a set of base domains to present as a floor for those to start with for an easy transition. Since the Gravity Project has done much of this work in defining and coding domains- their work is being used as the starting block. 

Introducing The Gravity Project

The Gravity Project is a collaborative of many who contribute information on screeners, their compatibility with questions to domains, and their coding crosswalks. This is immensely helpful when starting into the process of a CLRS. Our network can also join it, should we like to, and contribute to it! A list of the screeners and domains and coding inventories by them can be found here.
