      In consideration for being allowed to participate in any training, practice, tryouts, games, camps, or events organized by OBA – Ocala Basketball Academy ("OBA"), I, the undersigned participant (or parent/legal guardian if under 18), acknowledge, understand, and agree to the following:

      1. Assumption of Risk I am aware that participation in basketball-related activities involves physical exertion and inherent risks of injury, including but not limited to sprains, strains, fractures, concussions, permanent disability, and even death. I freely assume all such risks, both known and unknown, and take full responsibility for my participation.
      2. Release and Waiver of Liability I hereby release, waive, discharge, and hold harmless OBA – Ocala Basketball Academy, its directors, coaches, staff, volunteers, sponsors, facilities, and affiliates from any and all liability, claims, demands, or causes of action arising out of or related to any loss, damage, illness, injury, or death that may be sustained by me or my child while participating in or traveling to/from OBA-related activities.
      3. Medical Authorization I authorize OBA and its staff to obtain emergency medical treatment for me/my child in the event of injury or illness. I agree to be financially responsible for all medical treatment or transportation costs incurred.
      4. Code of Conduct I agree to abide by the rules and expectations of OBA and understand that poor sportsmanship, misconduct, or unsafe behavior may result in removal from the program without refund.
      5. Personal Property I understand that OBA is not responsible for any lost, stolen, or damaged personal property, including but not limited to athletic gear, clothing, phones, or valuables.
      6. Photography and Media Release I give permission to OBA to photograph or video record me/my child during events and activities, and to use those images for promotional or educational purposes without compensation.
      7. Legal Binding & Indemnification This agreement is legally binding and will remain in effect for the duration of my participation. I agree to indemnify and hold harmless OBA from any claims made by me or others resulting from my actions or participation.
      8. COVID-19 and Communicable Disease Disclaimer I acknowledge the contagious nature of COVID-19 and other communicable diseases and voluntarily assume the risk that I/my child may be exposed to or infected while participating in OBA activities. I agree that OBA is not responsible for any exposure, illness, or complications that may result.
      9. Refund and Cancellation Policy I understand that registration fees are non-refundable, including in the event of schedule changes, inclement weather, missed sessions, or early withdrawal from the program. Refunds may only be granted under exceptional circumstances at OBA’s discretion.
      10. Parental Supervision Clause (for Minors) For participants under eighteen, I acknowledge that I am responsible for ensuring appropriate transportation and supervision before and after scheduled activities. OBA is not responsible for children outside of designated training hours.
      11. Use of Outside Facilities I acknowledge that OBA may conduct practices, camps, or games at local gyms, parks, or rented venues. I understand that OBA is not liable for any hazards, defects, or incidents related to third-party facilities.
      12. Behavioral Expectations & Dismissal Clause I understand that any behavior by the participant or their family that is disruptive, disrespectful, unsafe, or negative impacts the team environment may result in immediate dismissal from the program, without refund.
      13. Compliance with Local Laws and RegulationsI agree that I/my child will comply with all federal, state, and local laws, rules, and regulations applicable to participation in the activities offered by OBA. Additional Legal and Risk Management Clauses
      14. No Guarantee of Results ClauseI understand and agree that participation in OBA programs does not guarantee athletic improvement, playing time, or placement on any specific team or scholarship opportunity. Results are based on effort, skill development, and individual progress.
      15. Force Majeure ClauseI acknowledge that OBA is not liable for delays, cancellations, or changes to programming due to circumstances beyond its control, including but not limited to natural disasters, pandemics, government regulations, facility closures, or acts of God.
      16. Voluntary Participation ClauseI confirm that I/my child is voluntarily participating in all activities, and I am not under pressure or obligation to enroll or continue participation in OBA programs.
      17. Dispute Resolution ClauseAny dispute or claim arising out of this agreement or related to OBA’s services shall be resolved through binding arbitration in accordance with the laws of the State of Florida. Each party is responsible for their own legal fees unless otherwise awarded.
      18. Health and Fitness DeclarationI certify that I/my child is in good physical condition and has been cleared by a medical professional to participate in high-intensity physical activity. I understand that it is my responsibility to notify OBA of any medical conditions, allergies, or injuries prior to participation.
      19. Technology and Communication ConsentI consent to receive email, text, or phone communication from OBA for program-related announcements, emergency alerts, and updates. I understand that I can opt out of promotional content at any time.
      20. Right to Refuse Service Clause OBA reserves the right to refuse participation or remove individuals from any program at its discretion for safety, behavioral issues, non-compliance, or any reason deemed disruptive or harmful to the environment.
