Instructions:

This application must be completed by anyone wishing to be a Support Volunteer at Moose Lake Gospel Camp. In order for MLGC to provide a secure environment both for the children it ministers to and also the leaders that minister, the following questions must be answered honestly and completely. Support volunteers are generally parent volunteers or maintenance volunteers

The MLGC Board Members/ Directors will keep all information strictly confidential. (Police may access this information, if requested.) Any questions of a sensitive or personal nature may be discussed with the MLGC Board/ Director upon completion and may not necessarily preclude your involvement at MLGC.

Liability and Health Release
You release Moose Lake Gospel Camp, its staff, volunteers, and board members from all manner of actions, claims, and demands of whatever nature resulting from any loss, injury, or expense sustained, during your attendance at Moose Lake Gospel Camp.

Media Consent & Release
I authorize Moose Lake Gospel Camp to take images or video footage of me, in whole or in part for various purposes. I waive any right to compensation, promotional material, and legal actions resulting from any media materials collected.

Commitment
Should my application be accepted, I agree to be bound by the Moose Lake Gospel Camp disciplinary processes if I am to break any MLGC policy or rule while serving at Moose Lake Gospel Camp

Note: If you are coming as a parent support staff in any capacity the camp would greatly appreciate your help to cover the cost of your stay. We suggest a donation of $125.00. This helps us cover the extra costs of food and utilities for the week. We are so grateful for all of our parent helpers who support us in helping run our camps, we do not want the suggested donation to stop you from joining us for the week therefore it is not mandatory. Donations can be made through e-transfer (mooselakegospelcamp@gmail.com), or paid the week of camp by cash, card or cheque.
