    The following info is requested by the Federal Government for certain types of loans related to a dwelling in order to monitor the lender's compliance with equal credit opportunity, fair housing, and home loan disclosure laws. You are not required to furnish this information but are encouraged to do so. The law provides that a Lender may not discriminate either on the basis of this information or on whether you choose to furnish it. If you furnish the information, please provide both ethnicity and race. For race, you may check more than one designation. If you do not furnish ethnicity, race, or sex, under Federal regulations, this lender is required to note the information on the basis of visual observation and surname if you have made this application in person. If you do not wish to furnish the information, please check the box, "I do not wish to furnish this information". (Lender must review the material to assure that the disclosures satisfy all requirements to which the lender is subject under applicable state law for the particular type of loan applied for.)

    Applicants should complete this form as a "Borrower", "Co-Borrower", or "Co-Signer". The Co-Borrower and/or Co-Signer information must also be provided (and the appropriate boxes checked) when the income or assets of a person other than the "Borrower" (including the Borrower's spouse) will be used as a basis for loan qualification or the income or assets of the Borrower's spouse or another person who has community property rights pursuant to State law will not be used as a basis for loan qualification, but his or her liabilities must be considered because the spouse or other person has community property rights pursuant to applicable law and Borrower resides in a community property State, the security property is located in a community property state, or the Borrower is relying on other property located in a community property State as a basis for repayment of the loan.
  • E-Sign & Disclosures and Consent

    Federal law requires you to receive certain disclosures and information about the products, services, or accounts you may receive or access in connection with your relationship with us ("Required Information"). With your consent we can deliver the Required Information to you by a) displaying or delivering the Required Information electronically, and b) requesting that you print or download the Required Information and retain it for your records. This notice contains important information before you consent to the electronic delivery of Required Information. Your consent also permits the general use of electronic records and electronic signatures in connection with the Required Information. After you have read this E-Sign & Disclosures and Consent, if you agree to receive Required Information from us electronically, and if you agree to the general use of electronic records and electronic signatures in connection with our relationship, please check the box  below acknowledging your consent. Statement of electronic disclosures: Call the Underwriter LLC and Think One Mortgage (NMLS125182), and its affiliates.  If you consent to electronic disclosures, that consent applies to all required Information we give you or receive from you in connection with our relationship and the associated notices, disclosures, and other documents. You agree to print out or download Required Information when we advise you to do so and keep it for your records. If you are unable to print or download any Required Information, you may call us and request paper copies. If you need to update your e-mail address or other contact information with us, you may do so by calling us and requesting the necessary updates. Your consent does not mean that we must provide the Required Information electronically. We may at our option deliver Required Information on paper. We may also require that certain communications from you be delivered to us on paper at a specified address. Withdrawing Consent: If you would like to withdraw your consent to receive future documents electronically and would prefer paper copies, please call us. If you withdraw your consent to receive documents electronically, the electronic service you are using will become unavailable to you, and your consent will not affect the validity or enforceability of prior electronic documents you received. I have read the information about the use of electronic records, disclosures, notices, and email, and consent to the use of electronic records for the delivery of Required Information in connection with our relationship. I will be able to view this information using my computer and software. I have an account with an internet service provider, and I am able to send e-mail and receive e-mails with hyperlinks to websites and attached files. I also consent to the use of electronic records and electronic signatures in place of written documents and handwritten signatures.
  • Alternate/Limited Document Loan Certification

    I have applied for a loan and have completed the borrower application containing various information regarding the purpose of the loan, the amount and source of the down payment, employment and income information, and assets and liabilities. I certify that all of the information is true and complete. I made no misrepresentations in the borrower application or in other documents, nor did I omit any pertinent information. I understand and agree that the lender, its successors, and/or assigns, reserve the right to change the loan review process to a full documentation program. This may include verifying the information provided on the application with the employer and/or financial institution. I fully understand it is a Federal crime punishable by fine or imprisonment, or both, to knowingly make any false statements when applying for this loan, under the provisions of Title 18, United States Code, Section 1014.
  • Acknowledgement and Agreement

    Each of the undersigned specifically represents to Lender and to Lender's actual or potential agents, brokers, processors, attorneys, insurers, servicers, successors and assigns and agrees and acknowledges that: (1) the information provided in this application is true and correct as of the date set forth in this application and that any intentional or negligent misrepresentation of this information contained in this application may result in civil liability, including monetary damages, to any person who may suffer any loss due to reliance upon any misrepresentation that they have made on this application, and/or in criminal penalties including, but not limited to, fine or imprisonment or both under the provisions of Title 18, United States Code, Sec. 1001, et seq.; (2) the property will not be used for any illegal or prohibited purpose or use: (3) all statements made in this application are made for the purpose of obtaining the financing of a loan; (4) the property will be occupied as indicated in this application; (5) the Lender, its servicers, successors or assigns may retain the original and/or an electronic record of this application, whether or not the loan is approved; (6) the Lender and its agents, brokers, insurers, servicers, successors and assigns may continuously rely on the information contained in this application, and are obligated to amend and/or supplement the information provided in this application if any of the material facts that have presented themselves have been changed prior to closing of the Loan; (7) in the event that my payments on the Loan become delinquent, the Lender, its servicers, successors, or assigns may, in additional to any other rights and remedies that it may have relating to such delinquency, report my name and account information to one or more consumer credit reporting agencies; (8) ownership of the Loan and/or administration of the Loan account may be transferred with such notice as may be required by law: (9) neither Lender nor its agents, brokers, insurers, servicers, successors or assigns has made any representation or warranty, express or implied, to me regarding the property or the condition or value of the property; and (10) my transmission of this application as an "electronic record" containing my "electronic signature," as those terms are defined in applicable federal and/or state laws (excluding audio and video recordings), or my facsimile transmission of this application containing a facsimile of my signature, shall be as effective, enforceable and valid as if a paper version of this application were delivered containing my original written signature. Acknowledgment. Each of the undersigned hereby acknowledges that any owner of the Loan, its servicers, successors, and assigns, may verify or reverify any information contained in this application or obtain any information or data relating to the Loan, for any legitimate purpose through any source, including a source named in this application or a consumer reporting agency.
  • Borrower Authorization & Privacy Notice

    I/We hereby authorize our underwriter and originator, and any of their successors, assigns, affiliates, investors, or service providers to obtain and verify information as necessary for processing my loan application. This includes, but is not limited to, verification of my employment history, income, bank accounts, investment and asset balances, credit history, current and past loans, and landlord references. I understand that this information may be obtained through third-party credit reporting agencies and other sources, and that a copy of this authorization may be accepted as an original. I/We further authorize the ordering of a consumer credit report to verify creditworthiness and authorize the use of any documentation obtained to process my application and evaluate eligibility for financing. This authorization will remain valid for 120 days from the date signed below. All information obtained will be used solely for the purpose of evaluating my eligibility for a loan and will not be disclosed outside the authorized parties except as permitted or required by law. You are not required to provide this information; however, failure to do so may delay or result in the denial of your application. This information is requested under applicable federal statutes including: Title 38, U.S.C., Chapter 37 (for VA loans) 12 U.S.C., Section 1701 et seq. (for HUD/FHA) 42 U.S.C., Section 1452b (for HUD/CPD) 42 U.S.C., Section 1471 et seq. or 7 U.S.C., Section 1921 et seq. (for USDA/FmHA)

    Please Note: If the Underwriter denies your application, you will NOT be charged the Underwriting Fee or Borrower Application Fee. However, if you are approved by the Underwriter and decide not to move forward with financing, you are obligated and agree to pay both the Underwriter and Application Fee within 5 working days from the approval notice from the underwriter. You have 60 Days to use your underwriter approval to find another mobile home within the same price range.
