Thank you for choosing our clinic for your healthcare needs. Our goal is to provide comprehensive, patient-centered care. Please review the clinic policies below before signing, as they outline important information about how our practice operates.
Appointments
Appointments are typically booked for 15 minutes. In a 15-minute appointment, we can usually address one main concern to ensure quality care. If you have multiple concerns, additional appointments may be required.
Appointment Punctuality
Please arrive on time for scheduled appointments. If you are more than 5 minutes late without prior notice, you will not be seen, and the appointment will be marked as a missed appointment. A $50 fee applies for missed appointments, late cancellations, or no-shows.
After-Hours Care
We offer same-week urgent appointments, 2 days every week (days will be posted on website). At this time, we do not provide after-hours coverage. If you require care outside of office hours, please visit urgent care or the ER.
Uninsured Services
Certain services are not covered by OHIP and will be billed according to the Ontario Medical Association (OMA) fee schedule. These may include:
- Doctor’s notes
- Prescriptions for massage/physiotherapy/orthotics
- Work or school forms
- Insurance forms
The turnaround time for forms can take up to 4 weeks.
Controlled Substances
Prescriptions for narcotics, benzodiazepines, and stimulants will be dispensed in 30-day supplies unless otherwise agreed upon. Patients on long-term controlled medications must attend an in-person review every 3 months before prescriptions can be renewed. We do not prescribe narcotics for chronic non-cancer pain, chronic anxiety, or chronic insomnia. Patients on long-term narcotics must sign a narcotics contract and may be required to undergo urine drug screening.
Respectful Conduct
All patients are expected to treat clinic staff—including physicians, nurses, and administrative personnel—with respect and courtesy at all times. Any form of verbal or physical abuse will not be tolerated and may result in dismissal from the practice.
Communication Protocol
To ensure smooth clinic operations: Excessive phone calls, emails, or messages that disrupt clinic workflow should be avoided. Urgent matters will be addressed according to clinic guidelines.
If patients have not heard a call back from the clinic regarding your results, call us back a week after doing your testing if you do not hear from the clinic.
Acknowledgment and Agreement
By signing below, you acknowledge that you have read, understood, and agreed to the clinic’s policies. Failure to comply with these policies may result in dismissal from the practice.
