YOU HAVE BEEN INVITED TO PARTICIPATE IN SHOOT 4 SIGHT, A SPORTING CLAY EVENT AND RELATED ACTIVITIES (THE “ACTIVITIES”) WHICH COULD SUBJECT YOU TO PERSONAL INJURY. TO PARTICIPATE, SIGN AND RETURN THIS FORM TO RELEASE AND INDEMNIFY VISABILITY.

In consideration for being permitted to participate in any way in "THE ACTIVITIES", I, for myself:

1. Hereby release, waive, discharge and covenant not to sue VisAbility or any sponsors or advertisers and their directors, officers, agents, and employees (herein referred to as “Releasees”) from all liability to the undersigned for any and all loss or damage, and  any claim or demands thereof on account of injury to the person or property or resulting in death of the undersigned arising out of or related to “THE ACTIVITIES”, whether caused by the negligence of the Releasees or otherwise.

2. Fully recognize that, in signing this release, if I am hurt or my property is damaged arising out of or related to “THE ACTIVITIES”, I will have no right to make a claim or file a lawsuit against the Releasees, even if they or any of them caused my injury or damage. 

3. Hereby agree to indemnify and save and hold harmless the Releasees and each of them from any loss, liability, damage, or cost they may incur arising out of or related to my participation in “THE ACTIVITIES,” whether caused by the negligence of the Releasees or otherwise.

4. Hereby assume full responsibility for any risk of bodily injury, death, or property damage arising out of or related to my participation in “THE ACTIVITIES,” whether caused by the negligence of the Releasees or otherwise.
