The name on both must match the name submitted on this form. This information is required for training eligibility and will be used solely for identity and license verification.

If all sessions are filled during that timeframe, I agree to choose from the available dates.

I understand that all deposits ($200 or full amount) are non-refundable, and the remaining balance (if applicable) must be paid no later than 7 days before the start of training.
I understand that any rescheduling must be requested with at least 10 days' notice. Last-minute cancellations may forfeit eligibility to rebook without a new deposit.
