By submitting this form, the participant and his/her/their guardian understand and accept the following terms and conditions for attending the New Frontier Jewish Youth Winter Shabbaton, February 28-March 2, being held at Congregation Netivot Shalom, Berkeley.
Hold Harmless
I hereby give permission to New Frontier Jewish Youth (NFJY), Congregation Sinai, and staff members for emergency transportation and/or treatment in the event of illness or injury, including first aid. I hereby accept responsibility for the payment of any emergency transportation and/or treatment. I further certify that I am fully competent and my child is in good physical condition, and have no medical or physical conditions that would restrict my child's participation in any program or activity, except those which were previously explained in the above medical information section. I sign below in affirmation and hereby release NFJY, Congregation Sinai, and staff members from any and all liability whatsoever.
Payment/Registration
- The payment of the deposit or full payment must be paid in full at registration.
- Withdrawal of enrollment 14 days-8 days prior the start of the program shall deduct 30% from the full amount provided to be returned to the child or guardian.
- Withdrawal of enrollment 7 days-0 days prior the start of the program shall result in no refunds.
- All medical conditions of the participant must be stated on the application form in a concise and clear manner.
Release of All Claims
- To the extent any participant takes part in any activity outside or not part of NFJY/Congregation Sinai's program, we shall not hold NFJY or Congregation Sinai staff responsible for anything that might happen to participants.
- NFJY/Congregation Sinai shall not accept responsibility for the loss or damage to any property belonging to the participants.
- NFJY and Congregation Sinai staff may take photos or videos while in the activities and may use such photos or videos for promotional purposes.
- The guardian authorizes NFJY/Congregation Sinai to seek medical treatment in the event of an accident or sickness. NFJY/Congregation Sinai will inform the parents/guardians of the participant in case such an event arises as soon as is practicable.
Participant’s Behavior
- The use of alcohol, illegal drugs, prescription drugs not prescribed to the teen, vape pens, edibles, or any other illicit item may not be brought to, used, shared, or encouraged at the event. Enforcement of this policy is for the safety of all our participants and will not be up for negotiation. Violation of this policy will result in your removal from our programming at the parent or guardian’s' expense.
- Bullying, harassment, assault, intimidation, or violence is unacceptable and will result in the participant’s immediate dismissal from our programming. We treat all with respect at New Frontier.
- I have read and agree to the New Frontier Code of Conduct.
I acknowledge that I have read and understand all of the above and that both I and the participant will comply with the rules and responsibilities set forth above.
