Upon completion of your application the next step in the application process is to complete the assessment testing. There are two assessment tests:
1. Math Assessment (7th – 9th grade math skills)
2. Journey Assessment (you will learn about the trade and answer some questions regarding it).
Completion of both tests are a requirement of the application process. Please call the Local 440 Training Center at (317) 856-6426 to schedule your test.

Required Documents
In order for your experience to be verified we will need the following documents included with your application. A picture or PDF of the information requested will work, please upload them below.
- High School Transcripts
  - High school transcripts with Diploma if graduation date is not listed on transcripts.
  - If you are currently a high school senior: High school transcripts with a letter from your school stating you are on track to graduate.
  - If you completed G.E.D. or High School Equivalency Program: High school transcripts, G.E.D. tests scores and certificate
- Current driver’s license
- OSHA 10 certification completed. (Optional)
- Service in the U.S. Armed Forces including discharge or release from active duty under honorable or general conditions (DD214). (Required IF you were in the Armed Forces)
