The DPW/NVO IMMEDIATE FEEDBACK FORM

WHO IS THIS FOR?
Any current member of DPW and NVO.

You can fill this out anonymously, although we won't be able to follow up with you if you do.

PURPOSE:
This is a way to give specific, relevant information about your experience to the DPW/NVO leadership team. We want you to be heard and to feel safe sharing information you think we should know.

Please focus on one topic per form and keep it focused on your experiences on crew this year.

Please speak from your own perspective and your own experiences. This is really important. We can't do much with hearsay. Please be as objective as possible, and also kind.

This is not for end-of-season / after-action feedback. We have the Exit Interview for that.

HOW LONG IS THIS FORM OPEN?
The Immediate Feedback Form will be open for the remainder of this season.

WHO DOES IT GO TO?
The output of this form will be available to every DPW and NVO Council Member and possibly shared with managers or others who can do something with it. We won't give out your name without your permission.
