The Surrender massage is not available to women over UK size 26 as certain aspects of erotic arousal can not be given. Instead, please book the Whisper or Ignite massage.
When completing the CAR form, please select all you want to experience in the massage. Some of the elements may need to be omitted, so if taking either the ignite or Whisper, note any aspects you particularly want prioritised.
Couples Massage
The Surrender Massage can also be given as the massage for the female partner when taking a Couples Experience Massage ( Watch & Enjoy or Watch & Touch or Give With Me ).
Completing the Consent Authorisation & Request Form
To ensure you receive the best experience, Gareth must understand your desires, expectations, boundaries and fixed limits. The CAR form has to be completed before you attend the appointment since, as well as understanding what you want to experience, it also gives Gareth consent to give this massage experience to you. Please be open and candid with your requirements and comments since the more Gareth knows your motivation, desires and limits, the better he can create your experience. During the massage, you are his total focus. You are guaranteed complete confidentiality and discretion.
Explaining the CAR Form 0 - 10 scale
Within the CAR form is a section listing the different erotic elements that can be included. By using this 0-10 scale, you can indicate which erotic aspects you particularly want to be included, might like to explore, or not to be included at all. Scores are not fixed and can be changed on the day by you by either talking through before the massage or indicating your movements during the massage. This allows you to explore something new that you may not have been able to experience before. If you score a ZERO, this is fixed and WILL NOT be included in the treatment.

Boundaries & Limits
Just before the massage begins, I always go over the more intimate aspects of the massage so you can reiterate any boundaries you want to be maintained. I operate a hand-holding/waving method that you can use to encourage something you are enjoying or stop if you are not comfortable. I am happy to use a 'safe word', but this must be discussed and agreed upon before the massage begins.
Other points
If you have not yet selected a date for your appointment, just put TBC (to be confirmed) in the date box. You can use a pseudonym (fake name), but I do need your real mobile number and email address as I will text or email you the appointment details and address details. This also authenticates you.

Cancellation
You can cancel your appointment at any time up to 24 hours before, and there is no cost. If you cancel within 24 hours, you have the choice to either rebook for another time and date or pay 50% of the fee. If you are a NO SHOW, you will be charged 100% of the fee.
AUTHORISATION STATEMENT
Completing this form acts as an authorisation by you to allow Gareth Millings and any associate he may work with to give this treatment to you, and by signing at the bottom of this form, you acknowledge that you fully understand the sensual and erotic nature of the treatment. You also understand that you can adjust your requirements anytime during treatment, but this must be clear to me. You also agree that the content of your appointment will remain confidential and will not be provided to any third party (apart from your partner or someone you wish to recommend the treatment to) unless Gareth Millings has given permission.
INSURANCE & ACKNOWLEDGEMENT
Gareth Millings has Civil Liability Insurance (including Public/Products Liability and Professional Indemnity) with BGI Insurance. This insurance covers his services in sensual massage, sensual massage classes and workshops, online and offline teaching of sensual massage.
If you are taking a sensual massage or class or workshop with Gareth then by completing this form, you are acknowledging that you are taking this with the full knowledge of the erotic nature of this massage/experience and on the day of your booking before taking this treatment you will be asked to sign an acceptance of service form before and after your treatment.
