As the Supervising Instructor/Trainer Please fill in the Apprenticeship form below with your observations regarding the First Aid Instructor hopeful's performance in each Competency area. Review with the apprentice and send in with the test sheet for the course attached. Rate performance based on the following:

No- The standard (must sees) have not yet been met. Any competancy answered with a NO requires further information in the comments box, and pathway reccommendations at the end of the document.

Shows Instructor responsibility to relevant safety regulations, and facility specific emergency procedures and response.
