As the Supervising Examiner Please fill in the Apprenticeship form below with your observations regarding the Examiner hopeful's performance in each Competency area. Review with the apprentice and send in with the test sheet for the course attached. Rate performance based on the following:

yes - They have completed the Examiner skills to the standard (the must sees) outlined in the Examiner Award Guide.

No- The standard (must sees) have not yet been met. Any competancy answered with a NO requires further information in the comments box, and pathway reccommendations at the end of the document.
