    Overview

    This registration form is only to be used by those families who have completed at least one year and will continue in the 2025-2026 school year in a Catholic school or hybrid school grounded in the Catholic faith (such as St. John the Baptist or Regina Caeli) or those who use an approved Catholic homeschool curriculum.  If your child meets one of these criteria and is preparing to receive First Reconciliation and First Communion in the 2nd grade or later or is preparing to receive Confirmation in the 8th grade or later at the end of the 2025-2026 school year, please register using this form. You may also register younger children for our Catechesis of the Good Shepherd program using this form. If your child is homeschooled, but you are not using an approved Catholic homeschool curriculum, please register for our Families of Faith or Confirmation Preparation program using this other form instead.

    Catechesis of the Good Shepherd (Sundays) - Montessori-style catechsis for children in PreK4 and K (Sunday mornings only).

    First Reconciliation and First Communion Workshops (Sunday and Wednesday) - Six workshops and a half-day Saturday Retreat (dates TBD).  

    Confirmation Retreat (Two Saturday Options Will Be Available) - Day-long Saturday retreat (dates TBD)

Once you have completed registration and submitted payment, you will receive confirmation of registration and payment from the CFF office to the email address you enter below.  There is a $75 registration fee per family which can be paid by credit or debit card, CashApp, or Afterpay (4 installments of $18.75).  If you are unable to pay online, please enter CASH in the coupon code box, and you will receive an invoice by email on how to pay by cash. If you are unable to pay the registration fee and need assistance, please email Ana Muina in our office at anam@stcatherinercc.org.

    • Emergency Medical Treatment Release: In the event of an emergency, by initialing below, I hereby give permission to transport my child(ren) to a hospital for emergency medical attention and grant permission for non-prescription medications to be given, if deemed appropriate.  (Any prescription medication to be given must be accompanied by a physician’s prescription or a parent note which should be provided separately).  I wish to be advised prior to any further treatment by the doctor and hospital.  If you are unable to reach me, my spouse, or the other designated guardian, contact the Backup Emergency Contact above.

      There is a $75 fee per family which can be paid by selecting one of the options below.  

      Credit Card - allows you to make a one-time payment using your credit or debit card.

      CashApp - allows you to make a one-time payment using your CashApp account.

      Afterpay - allows you to make four separate payments of $18.75 over a period of six weeks.

      You will select your payment method below.  If you choose to pay by CashApp or Afterpay, after you hit Submit Registration at the bottom, you will be taken to their external payment site to complete the transaction.  If you do not complete the payment, you will be able to return to this form and choose another payment.  If you do not receive an email verifying your submission, you will need to complete the registration again.
