UBSA Waiver & Release

By entering and using this facility and the equipment herein, and allowing my child(ren) to participate in any training, activities, camps, or use of video content provided by Uriah Bentick Soccer Academy (UBSA), I acknowledge and agree to the following:

Participation in sports training, fitness activity, and related programs involves inherent risk, including but not limited to sprains, falls, illness, injury, or equipment misuse.

I voluntarily assume full responsibility for any risks, injuries, or damages incurred by myself or my child(ren) in connection with participation in any UBSA program, on or off the facility premises.

I release UBSA, its owners, staff, volunteers, coaches, independent contractors, sponsors, and affiliates from any and all liability, claims, or demands arising from such participation.

I affirm that my child(ren) are in good health and physically fit to participate in UBSA activities.

I have consulted or will consult with a licensed medical professional if there are any concerns.

I understand that UBSA does not provide medical or health insurance coverage.

In the event of an emergency, I authorize UBSA staff to secure medical attention on my behalf and accept full financial responsibility for any associated costs.

I understand that UBSA may provide access to gym equipment, training tools, and video-based instructional content.

I agree that all participation is voluntary and done at my own risk.

I acknowledge that any instructional videos or digital resources provided are for informational purposes only and are not a substitute for professional medical advice, diagnosis, or treatment.

I understand that participation may involve exposure to illnesses, including but not limited to COVID-19. I voluntarily assume all associated risks and release UBSA from any liability related to such exposure.

I acknowledge that training, events, and other activities hosted by Uriah Bentick Soccer Academy may take place on private property owned by the Bentick family. By entering the premises and allowing my child(ren) and/or myself to participate:

I voluntarily accept all risks associated with being on the property, including but not limited to uneven terrain, natural outdoor elements, weather conditions, insects, and any use of facilities, fields, gym areas, and surrounding structures.

I agree to release, indemnify, and hold harmless the property owners (Uriah and Pamela Bentick), their family members, and any affiliated parties from any and all claims, injuries, damages, or losses arising from use of the property, including personal injury or property damage.

I understand that I am fully responsible for supervising any siblings or guests I bring to the premises who are not active participants in training.

I agree to respect all posted signage, fencing, restricted areas, and instructions given for safety.

I agree that I am entering and participating at my own risk and waive any right to pursue legal action against the property owners for any reason related to participation or presence on the premises.

I understand that Uriah Bentick Soccer Academy operates on private property, which may include a gym, open fields, and a designated playground area.

I acknowledge that all individuals—including parents, siblings, and guests—who enter the property or use the playground do so at their own risk, regardless of whether they are participating in any official training or activities.

I accept full responsibility for supervising any children or guests who are not actively participating in scheduled UBSA training sessions.

I understand that UBSA and the property owners are not responsible for injuries, accidents, or incidents that may occur in or around the playground, spectator areas, walkways, or general premises.

I agree to release, indemnify, and hold harmless UBSA, its staff, volunteers, and the property owners from any liability or claims related to unsupervised play, personal use of the playground, or general presence on the property.

UBSA may take photos or video recordings of training sessions, camps, or events for promotional, educational, and social media purposes.

By signing below, I confirm that I have read and understood this waiver and agree to all terms listed. I voluntarily agree to release and hold harmless Uriah Bentick Soccer Academy, its representatives, and facilities from all liability, claims, or demands arising out of participation in UBSA activities.
