Not every dog and dog owner pair is the right fit for puppy class lessons. Canine Academy must ensure that your safety, as well as the safety of your dog, our staff and all our human and canine customers, is prioritized. Please read the statements below and check the boxes to affirm your understanding and agreement with our requirements.
Owner Acknowledgement*

I understand and affirm that my dog is not reactive towards humans or other dogs, and bringing my dog to a group class environment will not put my dog, other people or other dogs at risk due to my dog's reactivity when in close proximity to these groups.I understand and affirm that I have the physical capabilities to maintain control of my dog on a leash at all times, and can prevent my dog from encroaching on the space of others, becoming loose or escaping while on any Canine Academy property.I understand that if either of the above statements proves to be untrue and/or if my dog's behavior is shown to be a poor fit for the group class environment, I may be asked to leave and considerations made for the tuition cost I paid Canine Academy.Finally, I understand that it puts my dog and others' dogs at risk to introduce unvaccinated dogs to each other, and therefore I affirm that my dog is now and will be kept fully up to date on their vaccinations while I am a Canine Academy customer.
