WAIVER AND RELEASE OF LIABILITY
In consideration for being allowed to participate in dance classes, rehearsals, and related activities (“Activities”) at Elite 16 Dance, I, the undersigned, on behalf of myself and/or my minor child, acknowledge and agree to the following:
Voluntary Participation: I understand and acknowledge that participation in dance involves physical activity and carries a risk of injury, including but not limited to sprains, strains, broken bones, or more serious injuries.
Assumption of Risk: I voluntarily assume full responsibility for any and all risks of injury or loss, including death, that may be sustained as a result of participating in these Activities, whether caused by negligence or otherwise.
Release and Waiver: I hereby release, waive, discharge, and hold harmless Elite 16 Dance, its owners, instructors, employees, agents, and volunteers from any and all claims, liabilities, damages, or causes of action for injury, loss, or damage to my person or property arising out of participation in the Activities.
Medical Treatment: I consent to receive medical treatment deemed advisable in the event of injury, accident, or illness during the Activities. I also agree to be responsible for any medical costs that may be incurred as a result.
Photo/Video Release: I grant permission for Elite 16 Dance to use photos and/or videos of myself/my child taken during the camp for promotional purposes (social media, website, flyers, etc.), unless otherwise stated in writing.
Behavioral Expectations: I acknowledge that respectful behavior is expected at all times. Elite 16 Dance reserves the right to dismiss a student from the program if their behavior is deemed unsafe or inappropriate, with no refund provided.
Refund Policy: I understand that registration fees are non-refundable.
