This form is to request an appointment only. Once submitted, I will contact you to go over the details and complete the booking process. To secure your spot a non-refundable $50 deposit is required. I understand the life happens if you need to cancel please do so at least 48 hours before your appointment. In that case your deposit can be applied towards rescheduling.
