Is your pet on their Rabies vaccination? If yes, you will need to show a Rabies Certificate at surgery check in. If your pet is not up to date, or if you cannot provide a Rabies Certificate the morning of surgery, you pet will receive a Rabies vaccine for an additional $6. This is non-negotiable as being current on Rabies vaccination is Ohio law.

Please Select
Yes, my pet is current on its Rabies vaccination. I will bring a Rabies Certificate the morning of surgery.
No, my pet is not current on Rabies vaccination and will need one for an additional $6
You will be contacted via e-mail to schedule your pet's appointment when we have an available surgery spot for your pet. You will then be given 48 hours to respond before we move on to the next person on the waiting list. Please check your email regularly, including junk/spam boxes, for communications from Rescue Village regarding this appointment.

I understand and will check my email, including spam/junk mail
I do not have email and will require a phone call
Unless I choose to directly transport my pet, I understand that on the morning of surgery my pet will be transported from the drop off location (Tractor Supply in Middlefield) to PetFix's clinic in Euclid, where they will be spayed/neutered and then transported back to the Middlefield location for me to pick up later that afternoon
*
I understand
I would rather drive my pet directly to PetFix in Euclid on the morning of surgery
I understand that my cat MUST be in a secure cat carrier when arriving for surgery (NO cardboard carriers permitted)
*
I agree
I do not agree and will seek services elsewhere
