  • In consideration for using RMC’s facilities as described above and RMC furnishing services and/or equipment to enable me to participate in activities associated with or enter upon the lands of RMC, their agents, owners, associates, and all other persons or entities acting in any capacity on their behalf, I hereby voluntarily agree to release, indemnify, discharge, hold harmless, and covenant not to sue RMC, on behalf of myself, my children, my parents, my heirs, assigns, personal representative, and/or estate for any and all claims of liability arising out of their negligence, strict liability, breach of contract, or any other act or omission which causes the undersigned illness, injury, death, and damages of any nature in any way connected with my participation in these activities or for being upon the lands of RMC.

    Medical information will be kept confidential and will only be accessible to Church Leaders, the Camp Directors, and our designated camp nurse. In the case of an emergency, information will be shared with medical personnel.

  • IMPORTANT: If you have been diagnosed with asthma by a physician and have medication including tablets, nebulizers, or inhalers, you MUST bring such treatment with you to camp or you will not be allowed to stay at camp! 

    If you have an inhaler, you must keep it with you at ALL TIMES during camp!

  • NOTE: For your own safety as a Leader/Counselor, please advise your Church Leader from your home church about your medication. Be discreet about your medication while at camp and secure it well within your luggage and out of sight at all times to ensure safety of campers and/or others.

  • I, (name if over 18 -or- parent or legal guardian if under 18 years of age) being aware of the penalty for perjury, do solemnly swear: I, individually and on behalf of my family, RELEASE : RHEMA Bible Church, RHEMA Bible Training College, RHEMA Bible College and Kenneth Hagin Ministries, Inc. (collectively referred to as "RHEMA") and their agents officers, directors, employees and instructors and Hudson Valley Family Church and their agents, officers, directors, and instructors and Refreshing Mountain Camp Inc. (RMC and their agents, officers, directors, employees, and instructors from all liability for any harm to myself/my child or my/my child's property resulting directly or indirectly from (a) my/my child's participation as an Encounter Youth Camp Leader or Counselor and (b) any harm to myself/my child or my/my child's property resulting directly or indirectly from any travel to, from or in connection with such participation. I personally assume all risks and liabilities in connection with my/my child's participation as an Encounter Youth Camp Leader or Counselor.

    I acknowledge and accept that there are significant elements of risk in any adventure sport, activity, or training that requires harnesses, belays, and other equipment or that may involve hiking, climbing, and/or repelling (i.e. Climbing Wall, Zip Lines, High Ropes Course, Climbing Tower, Leap of Faith, Giant Swing, Giant’s Ladder, Low Ropes, Swimming, Archery, Orienteering, Nature Studies, Physical Challenge Courses, Group Initiatives, and other events referred to hereafter as
    “activities”) and the use of any equipment.

    In consideration of being allowed to participate in any of the aforementioned activities at Refreshing Mountain Camp, Inc. (hereafter “RMC”) I, for myself and any minor children for whom I am parent, legal guardian, or otherwise responsible and for my / our heirs, personal representatives or assigns, hereby acknowledge the risks of injury or damage to property, personal injury, and or death involved in participating in the activities at RMC. I further agree to require said minor to participate in all safety training and wear all safety equipment provided by RMC for any of the above activities.

    I, for myself and any minor child(ren) for whom I am parent, legal guardian and/or otherwise responsible hereby release, acquit and forgive RMC, its principals, directors, officers, agents, employees, and volunteers (Releasees) from any and all liability of any nature for any and all injury or damage (including property damage, personal injury, illness, paralysis, and/or death) to me or said minor child(ren) as the result of my/our participation in the activities at RMC.

    I, for myself and any minor child(ren) for whom I am parent, legal guardian and/or otherwise responsible, and my/our heir(s) personal representatives or assign(s) also hereby expressly waive any claim, lawsuit, complaint, charge, or cause of action against RMC, its principals, directors, officers, agents, employees and for any and all injury or damage, to me or any such minor child(ren) and other person(s) as a result of my/our participation in the activities at RMC.

    I, for my/our heir(s) personal representatives, and assign(s) also hereby expressly agree to indemnify and hold harmless RMC, its principals, directors, officers, agents, employees, including costs, expenses, and counsel fees, from and against all claims, lawsuits, complaints, charges, or causes of action arising from the participation in the activities at RMC, and the activities for which this Release and Waiver agreement is given.

    I give permission for my child to ride in any vehicle designated by the Pastors, Youth Pastors, Leaders or Chaperones of RHEMA, Hudson Valley Family Church, and Refreshing Mountain Camp, its employees and adult volunteers, during and while at this event.

    I acknowledge that Hudson Valley Family Church and the Encounter Youth Camp reserves the right to use all photographs and video taken during the said event for future use.

    HEALTH CARE PROXY:
    In the event that I (or my child, if under 18 years of age) require health care decisions be made on my behalf, and if my emergency contact is unable to be reached, I hereby authorize any person who is 21 years of age or older and who is either an Encounter Youth Camp Leader or Counselor in an activity being held at Refreshing Mountain Camp, or holds a higher position, to be my health care agent and, following consultation with the attending physician of my agents choosing, to make any and all health care decisions for me/my child and hereby release this health care agent from any and all liability for the consequences of their decisions.

    RELEASE AND WAIVER OF LIABILITY AND INDEMNITY AGREEMENT

    THIS IS A CONTRACT AND AFFECTS YOUR LEGAL RIGHTS. PLEASE READ IT VERY CAREFULLY AND UNDERSTAND IT BEFORE YOU SIGN.

    I, the undersigned, hereby understand and unconditionally agree, on behalf of myself or, if I am signing this document on behalf of a person who is under the age of eighteen (18) (“Minor”), on their behalf as his/her Guardian, and my heirs, assigns, personal representatives and estate (or those of the Minor if I am his/her Guardian), to all of the terms set forth in this Release and Waiver of Liability and Indemnity Agreement (“Release”). This Release is for the benefit of Refreshing Mountain Camp, Inc. (“Refreshing Mountain” or “RMC”) and their respective directors, officers, employees, trusts and agents.

    DESCRIPTION OF THE ACTIVITIES. The activities, services, programs, and facilities at Refreshing Mountain provide opportunities for adventure recreation, including but not limited to ziplining, aerial excursion courses, elevated obstacle courses, high ropes courses, climbing, rappelling, giant swings (collectively, “aerial activities”), hiking, escape rooms, horseback riding, swimming and swim lessons, archery, slingshots, pedal carts, scavenger hunts, physical challenge courses, playgrounds, domesticated animal encounters, wildlife encounters, and other similar experiences.

    The aerial activities include varying combinations of zip lines, sky bridges, obstacles, elevated walkways, stairs, and hikes on uneven inclining and declining terrain. Participants may be exposed to sudden jarring impacts and sudden drops of height along the course. Participants wear safety harnesses and protective gear at all times. Aerial activities have a maximum weight limit of two hundred eighty (280) pounds. Participants must be five (5) years of age or older to participate in all other aerial activities.

    MEDICAL CONCERNS. Participants must be reasonably fit, must be able to demonstrate the required skill, and be able to understand all instructions prior to participating in any activity. Obesity, high blood pressure, cardiac and coronary artery disease, pulmonary problems, pregnancy, arthritis, tendonitis, prior head, neck, or back injuries or other joint and muscular-skeletal problems may impair the safety and wellbeing of participants during the activities, as may medical, physical, psychological and psychiatric problems. All such conditions may increase the inherent risks of the experience and cause the participant to be a danger to themselves and others. Participants with underlying medical problems that put them at greater risk of injury or illness during an activity must carefully consider those risks before choosing to participate, and they must fully inform the staff prior to the beginning of the activity. Refreshing Mountain reserves the right to exclude an applicant from participation for medical, safety, and/or other reasons.

    ACKNOWLEDGEMENT OF RISKS. (Initial each item below) IF I DO NOT AGREE WITH THE CONDITIONS BELOW, I WILL NOT USE, AND I WILL NOT LET MY CHILD(REN) USE REFRESHING MOUNTAIN’S FACILITIES.


  • I am physically and medically able to safely complete these activities. I am aware of the medical, weight and age restrictions to participate in the aerial activities, and I certify that I and/or any Minor accompanying me for whom I am Guardian, meet these restrictions. My participation in this activity(ies) is purely voluntary, and I have elected to participate in spite of the risks. I am not currently under the influence of alcohol, illegal drugs, or impairing legal drugs.


  • In consideration for using RMC’s facilities as described above and RMC furnishing services and/or equipment to enable me to participate in activities associated with or enter upon the lands of RMC, their agents, owners, associates, and all other persons or entities acting in any capacity on their behalf, I hereby voluntarily agree to release, indemnify, discharge, hold harmless, and covenant not to sue RMC, on behalf of myself, my children, my parents, my heirs, assigns, personal representative, and/or estate for any and all claims of liability arising out of their negligence, strict liability, breach of contract, or any other act or omission which causes the undersigned illness, injury, death, and damages of any nature in any way connected with my participation in these activities or for being upon the lands of RMC.


  •  I fully understand the activity or activities involve(s) risks and dangers of serious bodily injury, including permanent disability, paralysis, and/or death. These risks and dangers may be caused by my own actions or inactions, the actions or inactions of others participating in the activity or activities, the condition(s) in which the activity or activities take(s) place. I further recognize and agree to inspect each feature and associated facilities and equipment prior to use and read and obey all signs, and written or verbal rules associated with each activity. I further agree and warrant that at any time I believe conditions to be unsafe, I will immediately discontinue further participation in the activity or activities. There may be other risks and economic and social losses either known to me or not readily foreseeable at this time; and I fully accept and assume all such risks and all responsibility for losses, costs and damages I incur as a result of my participation, or that of the Minor, in the activity or activities. 


  •  I fully understand and acknowledge that: (A) activities at RMC have inherent risks, dangers, and hazards and such may exist in my use of RMC property and equipment and my participation in the above mentioned activities; (B) my participation in such activities and/or use of such equipment may result in injury or illness including, but not limited to bodily injury, strains, fractures, partial and/or total paralysis, other ailments that could cause serious disability or death; exposure to insect, tick, or snake bites, exposure to extreme temperatures and inclement weather, slips and falls, encounters with animals, collisions with other participants or objects, my physical condition, and injury and illness; (C) these risks and dangers may be caused by the negligence of the owners, employees, officers and agents of RMC, the negligence of participants, the negligence of others, accidents, breaches of contract, the forces of nature or other causes. If I choose to participate in RMC activities, risks and dangers may arise from foreseeable or unforeseeable causes including, but not limited to, misjudgment of a guide or employee, weather, land or course conditions, my ability to participate in the activity, risks of falling from high platforms, stands, or towers, risks of becoming entangled or ensnared with ropes or other equipment, and other hazards and dangers that are integral to or reasonably foreseeable recreational activities that takes place in an outdoor farm or recreational environment; and (D) by my participation in these activities and/or use of equipment, I hereby assume all risks and dangers and all responsibility for any losses and/or damages, whether caused in whole or in part by the negligence or other conduct of the owners, administrators, directors, agents, officers, members, volunteers or employees, other participants, sponsors, advertisers, and if applicable, owner and lessors of premises on which the activity or activities of RMC take place, or by any other person, and that they may arise from foreseeable or unforeseeable causes.  


  •  I hereby grant permission to RMC to make and use for promotion or other purposes, photo, video, and/or audio records of my and/or my child(ren)’s likeness and/or voice without recourse or compensation to me or anyone else otherwise. 

  • I HAVE READ THE ABOVE WAIVER AND RELEASE OF LIABILITY AND BY SIGNING IT, AGREE IT IS MY INTENTION TO EXEMPT AND RELIEVE RMC FROM LIABILITY FOR PERSONAL INJURY, PROPERTY DAMAGE OR WRONGFUL DEATH CAUSED BY NEGLIGENCE OR ANY OTHER CAUSE. I FULLY RECOGNIZE AND UNDERSTAND THAT IF I (OR ANY MINOR ON WHOSE BEHALF I AM SIGNING THIS RELEASE), AM HURT, DIE, OR MY PROPERTY IS DAMAGED, I AM GIVING UP MY RIGHT TO MAKE A CLAIM OR FILE A LAWSUIT AGAINST RMC EVEN IF THEY NEGLIGENTLY OR BY SOME OTHER ACT OR OMISSION CAUSE THE INJURY, DAMAGE OR DEATH. I EXPRESSLY ASSUME ALL RISK. I VOLUNTARILY SIGN MY NAME AS EVIDENCE OF MY ACCEPTANCE OF THE ABOVE PROVISIONS.

    I understand that this Release shall be governed under the laws of the Commonwealth of Pennsylvania. Any legal action arising hereunder shall be brought and decided exclusively by binding arbitration. If the Court finds any provision of this Release, or portion thereof, to be unenforceable, that provision of the Release will be enforced to the maximum extent permissible so as to effectuate the intent of the parties, and the remainder of this Release will continue in full force and effect. I understand that RMC reserves the right, in its sole discretion, to accept or deny services to any person that its owners, agents, or employees deem a hazard to themselves or to others. RMC may terminate my participation in the activity, if it believes me to be incapable of following the instructions, meeting the safety requirements or the rigors of participating in the activity, including but not limited to, any medical or safety reasons. I specifically agree to release RMC from any liability if I am prevented from participating in the activity for any reason whatsoever. RMC may alter its published or announced requirements for participation in its activities and for use of its property at any time and for any reasons that it may deem appropriate.

  • Please read the Code of Conduct in its entirety. If you agree and are willing to comply with all of the expectations of the Code of Conduct and Summer Leadership Camp Program, please sign at the bottom of the page.

     

    Out of our resolve to glorify God and respect each other, the Camp Leadership has established this Code of Conduct in an effort to model a portrait of Godly character while at Encounter Youth Camp.


    The following will be agreed to by each participant of the Encounter Youth Camp: 

    GENERAL
    -Have a positive attitude.
    -Honor and respect the Camp Leadership.
    -Abide by the camp schedule.
    -Never use foul language.


    ELECTRONICS, PHONES, Etc…
    Leave ALL electronics, devices, cell phones, etc. with group leader while at camp. They are not permitted. RMC Camp, Encounter Youth Camp and individual churches are not responsible for loss or damage to
    these items of value.

    GENERAL ATTIRE
    -Dress Modestly. If asked by a leader to change what is worn, participant will accept and comply without disrespect.
    -Excludes (but not limited to) the wearing of halter tops, strapless or tube tops, crop tops, spaghetti straps, off the shoulder shirts, muscle shirts, short shorts, cheerleading shorts, pants or shorts with writing on the backside, “sagging”- showing your boxers, low riding pants, etc.
    Leggings or biker shorts can be worn, but backsides should be covered.
    -Tank top straps should be two-finger width. Shorts should reach fingertip length.
    -Shirts and shoes must be worn in the dining hall and meeting rooms.
    -Shirts must be worn in the gym.
    -No pajamas in public settings.
    -Sneakers must be worn for all physical activities.


    POOL ATTIRE & CONDUCT
    -Respect the RMC lifeguard, RMC Staff and/or RMAI Camp Leaders. Obey their direction and all RMC pool rules, whether stated in the Code of Conduct and/or at RMC.
    -Enter pool area when lifeguards are on duty and it is the designated time on the camp schedule.
    -Wear a cover-up and/or t-shirt as well as shoes to and from the pool at all times.
    -Only swimming attire is permitted in the water. A dark t-shirt is permitted over swimsuits in water.
    -Girls/Ladies - One-piece swimsuit for all ages; certain tankini-style suits are permitted provided that no midriff is showing at any time.
    -Boys/Men - must wear boxer style suits. No Speedo suits.
    -No rough-play, no climbing on pipes or equipment, no shoulder chicken fights, no “dunk party”. You must be able to swim to use slides or diving boards.


    SAFETY
    -Exercise caution and good judgment in all areas of participation and activities.
    -Use sunscreen and bug spray as necessary; drink water frequently and regularly.
    -Report any and all illness or injury to a counselor.
    -Don’t engage in any form of fighting or role-play fighting. Damage caused by pranks and/or rough play will be billed to the responsible participant’s church group.

    STANDARDS FOR RELATIONSHIPS
    -Be kind and polite. Be a positive part of team/group activities.
    -Do not participate in any kind of hazing.
    -Campers and Counselors must refrain from PDA (public displays of affection) while traveling to and from and while at Encounter Youth Camp.

    CAMP LIVING
    -Keep the property clean. Do not litter (i.e., use trash receptacles).
    -Plants and animals are not to be removed from the premises for the preservation of the natural environment.
    -Immediately report damages to property and/or broken equipment to Encounter Youth Camp Leadership.
    -Be careful in “hot spot” areas: fire extinguishers, gym lights, holes in walls of bunks/meeting rooms, If RMC STAFF does any cleanup/repairs in those areas, our group will be charged for damages. If we know who is responsible for the damage, the participant’s church group will be billed for repairs.
    -Obey RMC QUIET HOURS (11:00 p.m. to 7:00 a.m.) which is strictly enforced by all Camp Leaders. (RMC will charge a monetary fine for violating this rule.) Obey CAMP QUIET ZONE (12:00 - 7:00 a.m.) within bunkhouse.
    -Be in dorm at designated times and stay in assigned bunk.
    -Don’t wrestle, fight, jump off bunks or run in the cabin/bunk house. Any and all damages will be billed to your church youth group if responsible for damage.
    -Comply with “lights out”.
    -Maintain personal hygiene daily, including the use of deodorant or antiperspirant.
    -Keep track of personal belongings. Be tidy.
    -Clean the bunkhouse as directed by camp leaders (throughout the week). Check-Out Day will require bunks to be left better than they were found and trash to be placed outside bunkhouse doors.


    DISMISSAL FROM CAMP – The following will result in parent/guardian being called and participant being dismissed from camp:
    -Stealing or snooping around another’s personal property.
    -Possession or use of any fireworks.
    -Possession and or use of alcohol, illegal drugs, tobacco and marijuana products (includes both smoking and e-cigarettes/vaping), etc.
    -Possession and/or use of weapons of any kind (including any/all knives: pocket, jack, Swiss army,...)
    -Leaving camp property without consent. Sneaking out of bunkhouses after curfew and/or during
    -Quiet Zone Hours.
    -Inappropraite displays of affection (i.e. kissing, etc…).
    -Entering the opposite sex’s dorm/bunkhouse/room/area.
    -Disrespect of Camp Leaders (either “continuous” or severe “one time”).

    The preceding Code of Conduct must be agreed upon. If, for any reason, the Encounter Youth Camp Leadership, Refreshing Mountain Camp personnel, and/or or other Rhema Church staff and/or Camp Leadership deems that a camper is not cooperating in accordance with this Code of Conduct, parents/guardian will be called and must make immediate arrangements to pick up camper from Refreshing Mountain Camp. (No refund will be given.)
