Please complete the following form, and a team member will follow up with you within 72 business hours with preliminary information about your coverage as well as next steps. Please note that it may take up to 14 business days to get a full verification back from your insurance.
The timeframe for appointment booking is longer for insurance coverage than it is with the direct-pay programs. Insurance verification takes time, and there are few slots available because the testing is extensive. It is likely to be at least one month between the insurance verification and the scheduled appointment date.
First available appointments for insurance-covered assessments are expected on or around June 1, 2025.
You can fill this form in regardless of what insurance you have, and we will follow up with information regarding if we are in network, if we have a clinician covering your state who is in network and when we plan to have a contract if we are not currently in network.
We are currently in-network with:
- Cigna
- Aetna
- United Healthcare
- Blue Cross Blue Shield
- Oscar
- Oxford Health
- Priority Health (Michigan)
- AvMed
- Medicare
We expect Medicaid coverage in all states to take one year or more.
We do not have coverage with all plans in all states, and we will notify you of the timeline for coverage if we do not have coverage in your state.
Please note, testing is only available for ages 18+ for ADHD and 21+ for autism.
Additionally, due to high deductibles and often low coverage for these services, there are situations where the amount owed through your insurance will be more than the out-of-pocket cost for the direct pay assessment. This is the due to requirements for testing that are more extensive for insurance coverage. This will vary widely by insurance plan, but it is possible.
