We are so excited for you to join us for the Winter/Spring 2026 semester!

Please fill out the enrollment form for all students and submit.

If you are re-enrolling, please check with Ellie if you would like to change your day to make sure there is space on your desired day, otherwise please select the day you have already been attending. By putting your signature on these forms, they are binding and you are agreeing to all policies (including payment policies) set in our Parent Handbook. Please review thoroughly. The non-refundable supply and registration fee will be due on submission of forms with no exceptions. Please venmo @ElliePropert or coordinate with Ellie (931-981-5811) on cash/check payment if you do not wish to pay through Venmo. Your slot is secured when payment is received.

If you need to withdraw from the program for any reason after submitting these forms, please let us know ASAP so we can allow another family an opportunity to enroll in the program. By signing these forms you agree to pay the non-refundable supply and registration fee due on submission of forms, even if you choose to withdraw from the program before your start date.

If you have worked with me before in a previous setting, please note that the prices are not the same. I am doing my best to make sure this is accessible and affordable for families. We are privately owned, operated, and self-sustaining and do not receive government assistance or funding. We are proud to be free from governmental influence, pressure, and mandates. This means that we do not receive grants that traditional 501-c3 educational groups receive. Please view our websites for more details on why we choose not to operate as a non-profit under the "financial assistance" page. If you are in need of financial assistance please submit the form on the website.

Please note: We would like to request that you help us accommodate your child by letting us know if there have been events in their life, past or present, that have caused an internal or external stressor. Our teachers are highly trained to work with children from all walks of life and are very familiar with behavioral presentations that occur as a result of traumas in the past, as well as appropriate ways to help a child regulate and overcome obstacles. Some of these backgrounds include a past in foster care, adoption, abuse, traumas (physical and emotional), neurodivergent tendencies, or bullying. We ask for this information to better help your child thrive during the school year. If you would like to discuss this in person with a teacher, please let us know in the paragraph below. If you are comfortable describing your child's needs in writing, please use the space below to tell us.

This health history is correct and accurately reflects the health status of the student(s) to whom it pertains. The person described has permission to participate in all activities except as noted by me, the parent or legal guardian. I give permission to the school staff to call EMS if a situation requiring emergency services occurs involving my child.

If I cannot be reached in an emergency, I give permission to the emergency contact on my forms to make decisions regarding my child's well-being. If child's parent or emergency contact cannot be reached, I give permission to school staff to make decisions regarding my child's well being until I can be reached. I understand that school staff will discuss with me prior to enrollment my preferences surrounding my child's health.

Release and Waiver of Liability and Indemnity
(Read Carefully Before Signing) This agreement by and between Shade Tree Nature School (hereinafter known as "Shade Tree") and the undersigned participant/trainer/instructor/employee/student/camper (hereinafter referred to as "participant") or the undersigned participants legal representative, parent, or guardian if such participant is a minor or does not have the legal capacity to enter into this contract is entered into for good and valuable consideration, the receipt and sufficiency of which is hereby acknowledged, and is subject to the following terms and conditions.

In consideration of being permitted to participate in any way in Shade Tree's tutorial, Camps, Programs, or Events and/or being permitted to enter for any purpose any restricted area (here in defined as any area where in admittance to the public is prohibited), the participant, named below agree:

1. The parent(s) and/or legal guardian(s) will instruct the minor participant that prior to participating in Shade Tree, Tutorial, Camps, Programs, or Events, he or she should inspect the facilities and equipment to be used, and if he or she believes anything is unsafe, the participant should immediately advise the officials of such condition and refuse to participate. I understand and agreed that, if at any time, I feel anything to be UNSAFE, I will immediately take all precautions to avoid the unsafe area and REFUSE TO PARTICIPATE further.

2.I/WE fully understand and acknowledge that: (a) There are risks and dangers associated with participation in Shade Tree, Tutorial, Camps, Programs, or Events which could result in bodily injury, partial and/or total disability, paralysis and death. (b)The social and economic losses and/or damages, which could result from these risks and dangers described above, could be severe. (c) These risks and dangers may be caused by the action, inaction or negligence of the participant or the action, inaction or negligence of others, including, but not limited to, the Releasees named below. (d) There may be other risks not known to us or are not reasonably foreseeable at this time.

5.I/WE HEREBY RELEASE, WAIVE, DISCHARGE AND COVENANT NOT TO SUE Shade Tree, including its owners, managers, promoters, lessor of premesis, lessees of premises used to conduct Shade, Tutorial, Camps, Programs, or Events, and event inspectors, underwriters, consultants and others who give recommendations, directions, or instructions to engage in risk evaluation or loss control activities regarding the Shade Tree Tutorial, Camps, Programs, or Events held at such facility and each of them, their directors, officers, agents, employees, all for the purposes herein referred to as "Releasee" FROM ALL LIABILITY TO THE UNDERSIGNED, my/our personal representatives, assigns, executors, heirs and next to kin FOR ANY AND ALL CLAIMS, DEMANDS, LOSSES OR DAMAGES AND ANY CLAIMS OR DEMANDS THEREFORE ON ACCOUNT OF ANY INJURY, INCLUDING BUT NOT LIMITED TO THE DEATH OF THE PARTICIPANT OR DAMAGE TO PROPERTY, ARISING OUT OF OR RELATING TO THE EVENT(S) CAUSED OR ALLEGED TO BE CAUSED IN WHOLE OR IN PART BY THE NEGLIGENCE OF THE RELEASEE OR OTHERWISE.

6.I/WE HEREBY acknowledge that THE ACTIVITIES OF THE EVENT(S) ARE VERY DANGEROUS and involve the risk of serious injury and/or death and/or property damage.

7.EACH OF THE UNDERSIGNED further expressly agrees that the foregoing release, waiver, and indemnity agreement is intended to be as broad and inclusive as is permitted by the law of the Province or State in which the event is conducted and that if any portion is held invalid, it is agreed that the balance shall, notwithstanding continue in full legal force and effect.

8.On behalf of the participant and individually, the undersigned partner(s) and/or legal guardian(s) for the minor participant executes this Waiver and Release. If, despite this release, the participant makes a claim against any of the Releasees, the parent(s) and/or legal guardian(s) will reimburse the Releasee for any money which they have paid to the participant, or on his behalf, and hold them harmless.

I HAVE READ THIS RELEASE AND WAIVER OF LIABILITY, ASSUMPTION OF RISK AND INDEMNITY AGREEMENT, FULLY UNDERSTAND ITS TERMS, UNDERSTAND THAT I HAVE GIVEN UP SUBSTANTIAL RIGHTS BY SIGNING IT, AND HAVE SIGNED IT FREELY AND VOLUNTARILY WITHOUT ANY INDUCEMENT, ASSURANCE, OR GUARANTEE BEING MADE TO ME AND INTEND MY SIGNATURE TO BE COMPLETE AND UNCONDITIONAL RELEASE OF ALL LIABILITY TO THE GREATEST EXTENT ALLOWED BY LAW. Photo Release and Consent

Please note that we do our best to make sure that faces of children are not seen in our advertising as we wish to protect privacy. However, we do ask that you fill out the consent or non-consent form in case there is media that shows the faces of students. If you choose non-consent we will not post pictures or videos on any platform exposing your child's face.
This agreement by and between Shade Tree Nature School (hereinafter known as "Shade Tree") and the party who signs, participating as a trainer, instructor, teacher, employee, volunteer, student or camper (hereinafter referred to as "participant") or participants legal representative, parent, or guardian if such participant is a minor or does not have the legal capacity to enter into this contract is entered into for good and valuable consideration, the receipt and sufficiency of which is hereby acknowledged, and is subject to the following terms and conditions.

I hereby grant to Shade Tree and their legal representatives and assigns, the right to photograph, videotape, and/or record my or my child/children's voice and sounds and to use any or all such photographs, recordings, and reproductions thereof in and/or as a part of any video production, broadcast, published products related to advertising, social media, trade show displays, and in any other purpose and in any manner and medium; and to edit the same without restriction.

I further grant the use of my name and/or my child/children's names in connection with my written or verbal comments and opinions. I hereby release Shade Tree and their legal representatives and assigns from all claims and liability relating to said photographs and video or tape recordings.

This release shall be binding upon the heirs and personal representative of the undersigned.

Withdrawal Policy
Non-refundable supply and registration fee is due within 3 days of submission of forms. By signing these forms you agree to pay the non-refundable supply and registration fee due on submission of forms, even if you choose to withdraw from the program before your start date.

First month's payment is due 4 weeks before school starts or upon enrollment if enrolling mid-semester. If you sign up mid-semester your invoice will be adjusted for any time missed.

Please note that withdrawal from any summer program at Shade Tree Nature School will require one month's notice. For example, if you plan to withdraw any day in November, we need written notice by October 1st. November’s tuition would be non-refundable. This gives us time to fill your child’s slot.

If you do not provide notice that you are withdrawing you will still be required to pay tuition until you provide withdrawal notice.

This policy will remain in place if you sign up mid-semester.

Please allow us ample time to fill your spot should you need to withdraw from any programs at Shade Tree.

We understand that emergencies happen and are happy to discuss withdrawal policy with you should there be an extenuating circumstance.

By signing these forms you agree to the terms of payment and withdrawal policy.
