Mini Apple Kitchen, LLC Release and Waiver

My child,  {minorChilds}, has my permission to participate in Mini Apple Kitchen, LLC cooking classes ("Classes"). I understand and accept that my child will be exposed to a variety of foods and that despite taking precautions, it is possible that my child's food items will come in contact with other food products, including allergens. In addition, I understand that my child will be working with cooking tools, knives, and appliances.

In consideration for my child being permitted to participate in Classes, and for other good and valuable consideration, I agree for myself and on behalf of my child (and all of our personal representatives, heirs, administrators, agents, and assigns) to release and discharge Mini Apple Kitchen, LLC (and its current or former affiliates, employees, agents, representatives, successors, franchisor, and assigns) from any and all liability, claims or causes of action (known and unknown), including any liability, claims or causes of action arising out of the negligence of Mini Apple Kitchen, LLC, whether active or passive, or any of its affiliates, employees, agents, representative successors and assigns. This waiver and release of liability includes, without limitation, injuries which many occur as a result of Mini Apple Kitchen, LLC's negligent instruction or supervision. I further expressly agree that the foregoing release and waiver agreement is intended to be as broad and inclusive as possible and that if any portion thereof is held invalid, I agree that the balance will continue in full legal force and effect. I have considered that if this waiver of liability was not as broad as it is, the cost for the Classes would be considerably higher, and as I do not wish to pay a considerably higher fee, I waive the right to bargain for different waiver of liability terms.

I ACKNOWLEDGE THAT I HAVE CAREFULLY READ THIS WAIVER AND RELEASE AND FULLY UNDERSTAND THAT IT IS A RELEASE OF LIABILITY, AND EXPRESS ASSUMPTION OF RISK AGREEMENT. I AM AWARE AND AGREE THAT BY EXECUTING THIS WAIVER AND RELEASE, I AM GIVING UP MY AND MY CHILD'S RIGHT TO BRING A LEGAL ACTION OR ASSERT A CLAIM AGAINST MINI APPLE KITCHEN, LLC FOR ITS NEGLIGENCE. I HAVE READ AND VOLUNTARY SIGNED THIS WAIVER AND RELEASE AND FURTHER AGREE THAT NO ORAL REPRESENTATIONS, STATEMENTS, OR INDUCEMENT APART FROM THE FOREGOING WRITTEN AGREEMENT HAVE BEEN MADE.

Participant's Release. I am freely signing this Waiver and Release. I affirm that I am at least 18 years old. I have read this form carefully and fully understand that by signing this form, I am giving up legal rights and remedies that may otherwise be available to me and my child.
