Skincare Treatments & Plans of Care

All skincare treatments provided are for cosmetic purposes and are not guaranteed to provide results for all clients, are not intended to cure, and are not intended to treat medical conditions. RN Mobile Skincare only treats healthy, uncomplicated skin to help reduce the signs of aging and to promote client confidence in skin appearance. Some clients may achieve results quickly, while others may require additional time and/or changes in their plan of care. RN Mobile Skincare does not provide any medical treatments without the collaboration of medical providers (physicians, dermatologists, etc.) and RN Mobile Skincare cannot be responsibile for any adverse reactions that may occur post skincare treatments. All pre-treatment and post-treatment instructions must be followed to maintain optimal results and to prevent adverse reactions. By initialing below, you acknowledge that you understand these terms and have been provided with pre & post-treatment instructions. 

Consent to Treat

All clients who book services & models for demonstration purposes must provide written consent for treatment. By initialing below, you are consenting to the assessment and treatment of your healthy skin. Any skin concerns that may require the expert consultation of a medical provider (like a dermatologist) will be noted, and a recommendation to follow-up with such provider will be made. RN Mobile Skincare cannot treat certain skin conditions (ezcema, psorisis, moles, rashes, vericose veins, etc.). By initialing below, you are consenting to the treatment of your healthy skin. 

Cancellations/Missed Appointments

An appointment represents time reserved personally for you and/or your party, and when appointments are made, they cannot be made for others during the same time. All appointments require a $100 booking fee which will be credited towards the cost of your treatments provided.  Cancellation/rescheduling of appointments must be done 24 hours in advance or the booking fee of $100 will not be credited to future services. By initialing below, you understand these terms. 

Release of Liability and Confidentiality

All clients receiving skincare treatments understand that their information will be kept confidential and will not be shared without their written consent (signed social media release form). We never use names in any social media posts, and your sensitive information such as address, phone number and/or birthdate will not be discolsed to anyone without your consent. 

All clients receiving skincare treatments understand that they are required to complete a new client evaluation form, which covers essential skin health information for safe and effective services. Ongoing services will not require a new form as long as there are no changes to the client's health status.

All clients receiving skincare treatments understand that their are no guarantees to treatment, and that all services provided may have varying results based on individual skin characteristics and responses to treatments. Although services provided require specialized training, are minimially invasive, and have over 10 years of clinical research with little to no negative reactions/results, not all individuals will acheive the results they expect and may require additional treatments, and/or additional consultation from other skincare experts for more advanced treatments (MedSpas, Dermatologists, etc.).

With all topical treatments, allergic reactions, sensitivities and mild discomfort, redness, inflammation and/or skin reactions may occur. If such reactions occur, you will be treated accordingly, including following pre-treatment and post-treatment instructions, and/or provided with recommendations for follow-up with expert consultants (NP, PA, MD, DO) for addiitonal treatments as needed. You will not blame, hold liable, or seek legal action against RN Mobile Skincare and/or Damion K. Jenkins, MSN, RN for unexpected reactions/results that may occur. 

All clients consenting to treatment understand and agree with all terms listed in this form. Anyone who does not consent and does not complete this form will not receive servies from RN Mobile Skincare and/or Damion K. Jenkins, MSN, RN.
