EMS Foundation
Welcome to the EMS Foundation Funding Request Form. Our mission is to support and enhance emergency medical services through funding initiatives that promote innovation, education, and community outreach. Eligibility Requirements
To be eligible for funding, applicants must meet the following criteria:
- Clearly demonstrate a need for financial support to enhance emergency medical services.
- Ensure alignment with the EMS Foundation's mission and values.
- Present detailed plans and measurable outcomes for the proposed project.
- While priority is given to funding requests from registered non-profits, it is not a mandatory requirement.

How to Apply
1. Ensure your request meets all the eligibility requirements.
2. Fill out all required fields with accurate and detailed information.
3. Include any necessary documentation such as project proposals, budget plans, and proof of non-profit status.
4. You will receive an autoresponse email notification that your application has been received and you will be contacted for further details if deemed necessary.
5. All applications are reviewed by the EMS Foundation Board of Directors. All decisions are at the discretion of the Board.
6. You will be notified by email within a month whether your funding request is approved.

If your organization or project is approved for funding, your signature below indicates your agreement to comply with the following conditions:
- Funds received will be used solely for the purposes specified in the application.
- Funds must be expended within one year of the date received. Any unspent funds must be returned to the EMS Foundation within one year of the receipt date.
- You must submit a brief project and financial report, along with receipts for expenditures, within one year of the receipt date.
- Your organization will acknowledge the EMS Foundation in associated media (print, signage, and/or digital) where possible. Please email us for our logo.
