Loaning Device and Digital Services Consent

Overview

ISD 709 Duluth Public Schools work to inspire every student to achieve their potential and prepare students to lead productive, fulfilling lives as citizens of Duluth and the wider world. It is important to maximize available resources as a wide range of educational opportunities will lead to lifelong learners and productive global citizens.

Educational technology resources support student learning and develop digital citizenship. In this effort, ISD709 provides the opportunity for all students to use district Chromebooks and approved software and online services. These resources are used in conjunction with the Electronic Technologies/Cyber Security expectations covered in the Family and Student Handbook and ISD709 Policy 524 Internet Acceptable Use and Safety Policy and 3187R Internet Use Regulations.

Parent/Guardian Consent for Digital Services
In order for ISD 709 Duluth Public Schools to continue to provide student(s) with the most effective tools to enhance their learning, teachers, assessment coordinators, and media specialists may use educational web-based programs and applications. Many of the educational web-based programs and applications require parent permission for children aged 18 and under.

Activities Permitted
Digital services must be used in a manner consistent with the educational mission and objectives of the Duluth Public Schools. Activities that are permitted include:

• School Work
• Original creation and presentation of academic work
• Research on topics being studied in school
• Research for opportunities outside of school related to community service, employment, or further education consistent with District requirements
  

Student Data Privacy Laws
We have a responsibility to protect our students' data and we closely follow federal laws to do so. These laws include FERPA (Family Educational Rights and Privacy Act), COPPA (Children's Online Privacy Protection Act) and CIPA (Children's Internet Protection Act).

Permission for student use of digital resources comes in two age groups. Please read carefully for information pertaining to your child's age.

Students Under the Age of 13 (COPPA) - District Approved Classroom Programs and Services

If you have a student who falls under the COPPA compliance age group (under 13), please review the Duluth Public Schools ISD 709 COPPA Compliance List that is attached on page 7 of this document(see page 7 of the PDF version). This list includes the names of the digital services used for educational purposes in our district along with links to their privacy policies and terms of service. Please review this list as these resources require parent permission for the personal information they may collect. By signing this document you are giving your permission for your child to use these services.

In order for our students to use these programs and services, certain personal identifying information must be provided to the website operator. This information can include, but is not limited to, the student's name, grade, birthday, email address/login, username, and teacher's name. ISD 709 shares minimum information with vendors. Throughout the school year in which this agreement exists, services may be removed from use and taken off the digital version of this form. No additional services will be added without individual parental consent.

Students Ages 13-18 - District Approved Classroom Programs and Services

If you have a student who is 13 or older, please review the Duluth Public Schools ISD 709 Age 13-18 Compliance List that is attached on page 12 of this document (see page 12 of the PDF version) located at the bottom of this document. This includes the digital services along with links to their privacy policies and terms of service. Please review this list as these resources may require parent permission for the personal information they may collect. By signing this document you are giving your permission for your child to use these services. If your student will be starting Kindergarten or 1st grade in the 2025-2026 school year, you can skip to the SIGNATURE portion of this form located on page 6.

 

Chromebook Loan Agreement

The Chromebook loan program is for students entering 5th through 12th grade, as well as students entering 2nd through 4th grade who take devices off campus in the event of remote learning and/or quarantine circumstances. This program is designed to improve student achievement by expanding and enhancing technology access, collaboration capabilities, and creativity for students in their learning.

Description of Loaned Property
The DISTRICT will provide to the student, according to the terms and conditions specified in this Agreement, the following device, equipment and accessories:

1. One (1) Chromebook
2. One (1) Power adapter

Throughout this Agreement, use of the term "device" is understood to include all of the items listed under "loaned property".

Chromebook Loan Agreement Terms

This Agreement will end on June 7, 2026, with the withdrawal of the student from the DISTRICT, or upon termination of this Agreement.

The DISTRICT agrees to:

1.  Provide a device in good working order upon delivery.
2.  Provide basic training on how to care, handle, and use the Chromebook for  learning.
3.  Provide basic device support and exchange of the device as available.

In the event of loss or damage of any kind to the Chromebook, the DISTRICT will, at its sole discretion, determine the cost and family's obligation associated with any damage to the Chromebook. We recommend using a protective case/sleeve for the Chromebook to help reduce the risk of damage. These situations may result in a temporary loss of access to a district device until the student's assigned device is repaired or an alternate loaning device becomes available.

The FAMILY and STUDENT acknowledges and agrees to:

1. Parents/Guardians are required to read this document and consent to the contained policies, by completing the necessary form, in order for their child to receive the device. It is encouraged that parents/guardians review this acknowledgement section with their child.
   
   
2. Each device has an identifying asset tag which must not be removed or altered. The device must remain free of all other writing, drawings, stickers or labels.
   
   
3. Use the device in a careful and proper manner that is strictly for district-supported curriculum.
   
   
4. The District reserves the right to repossess the device and accessories at any time if the student does not fully comply with the terms of this agreement. The District may also choose to limit and/or withdraw home use privileges for failure to comply.
   
   
5. The use of the device during instructional times is governed by the classroom teachers.
   
   
6. The device may ONLY be used by the student to whom it was assigned. The student may not loan it to another student unless directed by your school.
   
   
7. It is the responsibility of the student to keep their information secure.
   
   
8. Students are encouraged to store documents, worksheets, notes and other files to Google Drive; students are responsible for backing up and saving all work.
   
   
9. If the device needs to be restored due to technical difficulties or another reason, the District is not responsible for loss of any content put on the devices by the student such as music, video, photo, documents, etc.
   
   
10. Assume responsibility for supervising the student's use of the device at home, following the guidelines of the Family and Student Handbook.
    
    
11. The device is, and at all times shall remain, the sole and exclusive property of the DISTRICT, and the FAMILY shall have no right, title, or interest therein, except as set forth in this Agreement.
    
    
12. The FAMILY shall indemnify and hold harmless the DISTRICT and its agents against any and all claims, actions, suits, proceedings, costs, expenses, damages and liabilities, including attorney's fees and costs arising out of, connected with or resulting from the device, use of the device, or this Agreement, including, without limitation, the manufacture, delivery, possession, use, operation, or return of the device.
    
    
13. Return the Chromebook to your school or report the Chromebook as lost. Agree to pay all replacement or repair costs.

Theft: If you believe a school-issued device was stolen please refer to the following guidelines. On school grounds: Report theft to the Digital Innovation Specialist and building administration (Principal or Dean In some situations a police report may be filed. Outside of school: File a police report and report to the Digital Innovation Specialist.

 

*If your family is participating in the Educational Benefits Program, please contact your school Principal for a possible reduction, grant, variance, or waiver on these fees.

 

General Care Instructions

1. Each device has an identifying asset tag which must not be removed or altered. The device must remain free of all other writing, drawings, stickers or labels.
   
   
2. Screens on all devices should only be cleaned with a soft, clean cloth. Chemical cleaners or liquids, including water, should not be used on any device.
   
   
3. Students should not put weight on the device. They should not be stacked or wedged tightly into a backpack. The case or laptop should not be used as a folder or for holding other items.
   
   
4. Liquids, food, and other debris can damage the devices. Devices should be kept away from such material at all times.
   
   
5. Devices should not be exposed to extreme temperature conditions. Students should not leave a device in an area that would have temperatures below freezing or above 95 degrees. Devices should never be left in a vehicle overnight.
6. All repairs must be completed through the ISD 709 Technology Department. Due to warranty guidelines, students and/or their families must not complete or arrange for any repairs.

 Please review the following statements. Fill in your information, your child's information, and sign below to provide consent for your student to use the educational technology resources offered by ISD 709 for the 2025-2026 school year.

I consent and agree to:

Duluth Public Schools providing personal identifying information about my child to the software, websites, and web-based applications listed in this document.

Use the district Chromebook in accordance with appropriate behavior expectations as explained above (if applicable).

Understand that any violation of Chromebook Loan Agreement conditions may result in the removal of the Chromebook from the student (if applicable).
