Background Check & Fingerprinting Consent

Important: For the safety and well-being of our athletes and staff, and in compliance with relevant youth protection laws and best practices, a comprehensive background check and fingerprint screening are required for all potential trainers as a condition of engagement/employment with Big Batz LLC.

The background check will be conducted by a third-party consumer reporting agency in compliance with the Fair Credit Reporting Act (FCRA) and applicable state/local laws.
You will receive separate disclosures and authorization forms from the background check vendor, which may require additional information (such as your Social Security Number and Driver's License Number) submitted directly and securely to them. Do NOT include your SSN on this initial form.
Fingerprinting will be conducted via a designated Live Scan service provider. Instructions on scheduling this appointment will be provided if your application proceed.
Big Batz LLC. will cover the cost associated with the background check and fingerprinting process.
