I accept the above instrument with these accessories
Shoulder rest, bow, case
for the monthly amount of $20 due and payable on the agreed-upon day of each month by charge on the credit card on file.
I agree to pay a deposit in the amount of $200 which may be applied to any rental item replacement, or charges for repairs, if needed. I also agree to replace this deposit amount if it is used for these purposes, after receipt of a written notice. This deposit will be refunded to me at the end of the contract period after all rental items have been returned and after payment has been made for repairs, refurbishing and/or cleaning.
I, as renter, understand that I am responsible for notifying my property insurer for proper coverage to cover damage or loss of the rental items as I am hereby notified that LaMotte Violins does not insure them.
I may return these rental items at any time during the life of this contract at which time I agree to pay all charges that may become necessary in order to return the rental items in the same condition that they were initially rented on the date below.
I understand that LaMotte Violins agrees to apply 100% of the charges within the first 12 months towards any instrument or bow in the LaMotte Violins inventory.
Failure to comply with any of the contract agreements will be a breach of contract and LaMotte Violins may demand the return of all items and payment in full, for either the value of the items or the costs associated with returning them to their origianl state as at the commencement of the rental period. If I do not return the items listed within five days, the deposit may be applied and the credit card used to pay the balance of the listed collection costs.
I have read, and agree to all of the terms contained within this contract.
