Health Form
You will find the health form as a link on the 'Registration' section of the Summer Camp Site. The health form needs to be filled in and returned/emailed by the first day of summer camp. It is not necessary to fill one in, if the school already has a completed pupil health form for 2025-2026.The completed form should be returned to the school by email to kkonstantopoulou@campion.edu.gr.
Transportation
If you require transportation, you will find the 'Bus Request Form' on the 'Registration' section of the Summer Camp Site. Any questions about summer camp transportation, please contact our transportation officer, Ms Patouha on mariapat@campion.edu.gr or on 210 6071716.

Consent Form
On this page you will give your consent to certain aspects that will ensure that the summer programme operates at maximum efficiency. The electronic signature below and its related fields are treated by Campion School as a handwritten signature.
I understand that if, at the time of the summer camp, my child is taking any medication I must fully inform the Camp Director, Ms Angie Gargareta or Ms Konstantopoulou, the nurse. I understand that children are not allowed to hold medication in their possession and must hand it over to the Campion School nurse, at the start of each morning.

I have spoken to my child about the need to behave in a sensible, appropriate manner and I accept that if my child is unable to behave in an acceptable fashion then their continued place in the programme may be put at risk.

I understand that the programme begins at 9.00am and ends at 3.30pm. I understand that it is my responsiblity to get my child to Campion School on time and to be punctual when collecting my child (unless on the school bus)

I understand that by enrolling my child in the Summer Camp, I am authorising photographs/video of my child to be used internally within the Summer Camp (only first names will be used internally); and externally for recognition and future promotional material (At no point will the name of your child appear next to an image of your child).

Photos/videos for internal use during Summer Camp 2026: I give my consent for my child to have pictures and videos only to be used for summer camp purposes i.e. for summer camp class photos or class collages. To this effect these photos will be shared to other parents of summer camp attendees. Only first names will be used, no surnames. Photos and videos will not be used for wider promotional material. If you decline consent, your child will not be photographed or video-ed in any summer camp activity.

I recognise that, by its nature, participation in athletic activities includes risk of injury which may range in severity from minor to severe. Although serious injuries are not common in supervised athletic activities, it is impossible to eliminate the risk.
