Wavier and Release of liability for Children under 16 years old.

Activity: Participation in the fitness and recreational activities at 180 Fitness Hub
Waiver and Release of liability: I, the undersigned, as the parent or legal guardian of the above named child, acknowledge that I am voluntarily permitting my child too participate in physical activities at 180 Fitness Hub Limited.
I understand that there are inherent risks associated with physical activities, including but not limited to falls, contact with other participants and overexertion, which may result in injury or other consequences.
Release of liability:
in consideration of the benefits derived from participation in the activities at the facility I do hereby agree as follows:

1. Assumption of Risk: I acknowledge that I am fully aware of the risks and dangers involved in the activities my child will be participating in and voluntarily assume all risks associated with there participation.
2. Release: I release and hold harmless 180 Fitness Hub Limited, its owners, operators and staff from all liabilities, claims or demands that may arise from my child's participation in activities at the facility.
3. Medical Authorisation: In the event of an injury or medical emergency, I consent to the administration of first aid and/or emergency medical treatment from my child.
4. Photo Release: I grant permission for 180 Fitness Hub Limited to use Photographs or videos of my child taken during activities for promotional Purposes.
5. Compliance with Rules: I Agree to ensure that my child and I Have read and agree to comply with all the rules and regulations.
6. All Under 16 members must undertake and induction before the start their membership with 180 Fitness Hub Limited.

Acknowledgement of Understanding.
I have read this wavier and release of liability, and I understand its terms. I am signing this document freely and voluntarily, and I am aware that this wavier and release of liability will be binding upon me and my child heirs and assigns.
