Liability Release Form
The Etiquette Academy at Lakewood

In consideration of my child’s participation in The Etiquette Academy hosted by Lakewood
Country Club (hereinafter referred to as the “Club”), I acknowledge and agree to the following:
Assumption of Risk
I understand that participation in an etiquette class, although low-risk, may involve certain inherent risks including but not limited to slips, falls, allergic reactions (e.g., to food or materials), and interactions with other participants. I voluntarily assume full responsibility for any risks of injury, illness, or loss arising from my child’s participation in the class, whether caused by negligence or otherwise.
Release and Waiver
I hereby release, waive, discharge, and covenant not to sue Lakewood Country Club, its owners, officers, employees, volunteers, instructors, and agents (collectively, the “Released Parties”) from any and all liability, claims, demands, actions, and causes of action whatsoever arising out of or related to any loss, damage, or injury that may be sustained by my child while participating in the event(s).
Medical Consent
In the event of an emergency, I authorize the Club and its representatives/volunteers to seek medical treatment for my child at my expense. I understand that every effort will be made to contact me prior to such action.
Behavior Expectations
I understand that all participants are expected to behave respectfully toward others and follow instructions given by the class instructor(s). The Etiquette Academy at Lakewood reserves the right to dismiss any participant whose behavior is disruptive or unsafe without refund.
Indemnification
I agree to indemnify and hold harmless the Released Parties from any claims, damages, costs, or expenses, including attorney’s fees, arising from my child’s participation in the class.
I certify that I am the parent or legal guardian of the child applying to participate in The Etiquette Academy at Lakewood and have full authority to sign this waiver. By signing below, I understand and acknowledge that I have carefully read and fully understand all provisions of this Liability Release and waive all rights on behalf of myself and my minor child participating in the event(s).
