    THRYVE INITIATIVE PARENTAL CONSENT FORM

  • This consent form grants permission for the minor named below to participate in one-on-

    one virtual coaching sessions provided by THRYVE Initiative. These sessions are

    designed to support personal growth, emotional resilience, and life skills development in

    teenagers through non-clinical coaching methods. By signing this form, the parent or

    legal guardian affirms their understanding and acceptance of the terms outlined below

  • 1. Nature of Services

    Coaching services provided by THRYVE Initiative are educational and supportive in

    nature. These services are not a substitute for licensed therapy, psychiatric care, or any

    form of mental health treatment. Coaches do not diagnose or treat mental health

    conditions. Participation is voluntary and may be withdrawn at any time by notifying

    Thryve Initiative in writing.

  • 2. Confidentiality and Mandatory Reporting

    Coaching sessions are private and confidential. However, THRYVE Initiative is required

    by law to report any disclosures indicating that the participant is a danger to themselves

    or others, or if there is suspicion of abuse or neglect. In such cases, appropriate

    authorities or emergency contacts may be notified.

  • 3. Online Communication and Security

    Sessions will be conducted through secure, password-protected video conferencing

    platforms. While every effort will be made to maintain confidentiality, THRYVE Initiative is

    not responsible for breaches due to internet security issues or third-party platforms.Participants are expected to join sessions from a quiet, private space and use their own

    headphones when possible.

  • 4. Media Release

    With additional written consent, THRYVE Initiative may use photographs, video, or

    quotes from participants for promotional or educational purposes. No identifiable

    information will be shared without explicit permission.

  • 5. Code of Conduct

    Participants are expected to maintain respectful communication during sessions.

    Inappropriate behavior, including hate speech, harassment, or any form of disruption,

    may result in removal from the program.

  • 6. Emergency Protocol

    In the event of a medical or psychological emergency, THRYVE Initiative staff may

    contact emergency services and/or the parent or guardian listed on this form. It is the

    guardian's responsibility to ensure emergency contact information is up to date

  • 7. Limitation of Liability

    By signing this form, the parent or guardian agrees to release and hold harmless

    THRYVE Initiative and its coaches, staff, volunteers, and representatives from any

    claims, damages, or liabilities arising from participation in online coaching sessions,

    except in cases of gross negligence or intentional misconduct.

  • 8. Governing Law

    This agreement shall be governed by and interpreted in accordance with the laws of the

    State of Wyoming, without regard to its conflict of laws principles.

  • Consent and Acknowledgment

    I have read and understood the above terms. I voluntarily consent for the minor named

    below to participate in online coaching sessions with THRYVE Initiative.
