  • Please enter the scores for each of the three screening tools:

  • Universal                                         

    This applies for a physical activity METS score of more than 7.

    • Provide with local and tumour specific resources as available.
    • No assessment required.
    • Patient provided with universal prehabilitation.
    • Patient re-screened as required along cancer pathway.

  • Targeted - refer for assessment                              

    This applies for a physical activity METS score of 4-7.

    • Patient provided with universal prehabilitation.
    • Provide with local and tumour specific resources as available.
    • Onward referral for prehabilitation assessment.

  • Specialist - refer for assessment                              

    This applies for a physical activity METS score of less than 4.

    • Patient provided with universal prehabilitation.
    • Provide with local and tumour specific resources as available.
    • Onward referral for prehabilitation assessment.

  • Universal                                          

    This applies for a nutrition (PG-SGA) score of less than 2.

    • Provide with local and tumour specific resources as available.
    • No assessment required.
    • Patient provided with universal prehabilitation.

    This applies for a nutrition (PG-SGA) score of 2-3.

    • Patient provided with universal prehabilitation.

  • Specialist  - refer for assessment                            

    This applies for a nutrition (PG-SGA) score of 4 or more.

    • Patient provided with universal prehabilitation.
    • Provide with local and tumour specific resources as available.
    • Onward referral for prehabilitation assessment.

  • Universal                                           

    This applies for a psychological wellbeing (PHQ- 4) score of 5 or less.

    • Provide with local and tumour specific resources as available.
    • No assessment required.
    • Patient provided with universal prehabilitation.

  • Targeted - refer for assessment                            

    This applies for a psychological wellbeing (PHQ- 4) score of 6-8.

    • Patient provided with universal prehabilitation.
    • Provide with local and tumour specific resources as available.
    • Onward referral for prehabilitation assessment.

  • Specialist - refer for assessment                            

    This applies for a psychological wellbeing (PHQ- 4) score of 9-12.

    • Patient provided with universal prehabilitation.
