LIABILITY RELEASE - important please read:

I, the undersigned, am the parent or legal guardian of the above-named player, who is under the age of 21 years, and I do hereby consent to the player’s participation in the sport of soccer as a player with EMPOWER SOCCER (a program of Empower North County), and such participation includes but is not limited to all practice sessions, scrimmages and games.

Additionally, I do hereby release and forever discharge said EMPOWER SOCCER from any and all liability whatsoever and from any claim or any action or any claim for relief which may be asserted against said EMPOWER SOCCER or against any individual who is a member of said EMPOWER SOCCER including players as well as adults, by reason of any injury said player may receive or incur while participating in the sport of soccer and said soccer-related activities as set forth above or in the transportation of said player to or from any game or soccer-related activity.
 
Additionally, I hereby authorize EMPOWER SOCCER personnel to seek emergency medical and/or dental treatment for said player by a Doctor of Medicine or by a doctor of dentistry, so long as they are licensed to practice by any state of the United States, or to seek said treatment by any qualified paramedic or nurse. I agree to pay for said medical and/or dental treatment to the extent that any medical or dental insurance that may cover said player is insufficient.

Finally, I represent to EMPOWER SOCCER that I have, personal medical health insurance that will provide coverage for said player in case any accident or injury should occur relative to any practice, any game or any soccer-related activity. I further understand that having such medical health insurance is an express condition to said player’s participation with EMPOWER SOCCER and that the policy is in force and that such policy shall remain in force while said player is a member of any EMPOWER SOCCER activity or said policy shall be substituted for by another policy. I further agree to inform EMPOWER SOCCER of any changes to the above facts and information as quickly as possible.
 
I hereby acknowledge that it is my sole obligation to pay whatever expenses may be incurred relative to medical and/or dental treatment for said player to the extent that said family or personal medical insurance is not sufficient.
 
I hereby acknowledge that I have read all of the foregoing information and that I understand such information and I hereby acknowledge my consent to said player’s participation relative to all foregoing statements, representations and conditions.

PARENT CODE OF CONDUCT - important please read: 

Youth soccer is for the players. All parent, guardian, friend, or family members associated with Empower Soccer are invited to share in the pleasure of watching their children participate in the world’s most popular sport, so long as their behavior does not distract the players and officials from the free flow of play which is the trademark of the sport.
 
Empower Soccer places significant value on having respectful and engaged families, and we ask that parent, guardian, friend, or family members continue to adhere to these guidelines.
 
A parent, guardian, friend, or family member should:
- Foster an environment of respect for referees, players, coaches, and spectators of the opposing team through words of encouragement and support.
- Cheer on players from the sidelines – do not coach.
- Applaud superior play, effort, and sportsmanship by players on both teams.
- Support your coaches and managers consistently regardless of the results on the field. Coaches contribute many hours of their time to your children. They deserve your congratulations when the team wins and your encouragement when the team doesn’t.
- Communicate any concerns to the coach at the appropriate time away from the playing location and players (we recommended 24hr cool off time).
- Always remain at least two yards from the playing field so our young players have room to play and enjoy the game.
- NEVER coach or correct a player that isn’t your child.
- Your cooperation with these standards before, during, and after each game or practice will make Empower Soccer more enjoyable for everyone.
 
A parent, guardian, friend, or family member should:
- Never use foul language or obscene gestures at a game or practice site.
- Avoid comments and gestures that express disagreement with referee decisions.
- Cooperate with any request by the game officials.
- Never consume alcoholic beverages on the field location at practices or games.
- Never smoke (tobacco or marijuana) on the field location at practices or games.
 
A parent, guardian, friend, or family member should understand that:
- Any parent or spectator who fails to adhere to these standards will be required to leave the playing area, and play will be suspended until he or she does so.
- Empower Soccer reserves the right to suspend or terminate a player’s enrollment for his or her parent’s persistent or extreme sideline misbehavior, and there will be no refunds of player fees in such cases.
- Parents or guardians are responsible for their guests’ behavior and must inform their guests of the applicable rules of conduct.
 
Any breaches of this Code of Conduct can result in your child’s suspension and a member’s removal from Empower Soccer Club. Comments and behavior of any member which contradict the mission statement or disrupt the functioning of a team can result in the suspension and/or removal from Empower Soccer Club.
 
 
A parent, guardian, friend, or family member should:
- NEVER use foul language or obscene gestures at a game or practice site.
- Avoid comments and gestures that express disagreement with referee decisions.
- Cooperate with any request by the game officials.
- NEVER consume alcoholic beverages on the field location at practices or games.
- NEVER bring weapons (guns or knives) to any area of a park (public or not) where Empower Soccer has games. 
- NEVER smoke or vape (tobacco or marijuana or whatever else) at the field location of practices and/or games- .
 
A parent, guardian, friend, or family member should understand that:
- Any parent or spectator who fails to adhere to these standards will be required to leave the playing area, and play will be suspended until he or she does so.
